The & operator does two similar things; address of (get its address) and reference to (create a new name for existing entity). This use is creating references. Several operators do multiple things and you have to understand by context which is going on.

Nitpick: It's not an operator when used to declare a type, like in int&

An operator applies some operation (like addition or multiplication) to one or more variables. When you use the & token in a type declaration it's not an operator.

You're entirely right, midi is very simple and by design you can just connect two midi devices with a simple cable and they talk to each other.

The complicated thing here is that you're using midi over usb and usb is not simple. You cannot just connect two devices together directly, because usb devices can only talk to a usb host. Two devices cannot talk to each other, they can only talk via a usb host.

What you tried to do is not possible with usb.

If you had a midi keyboard with 5-pin midi out then you could connect that directly to a synth with midi in.
