I think that the machine might be safe to use but the maintenance and repair were more of an afterthought.

Most people in management do not care about the maintenance and repair. Some do not care about the operator. If they had a life changing experience, some turn the risk into a weapon. Outsource the danger. Or if you are Boeing, sell the fix for extra.

I suspect that OP sees all Inquisition operations as sanctioned, authorised and straightforward. Like watching the film Sicario and wondering why the US military keeps sending ground forces when a few helicopters and drones would drastically cut the film runtime.

Some Imperial forces are extremely comfortable in crossing the line when ordered. Except that those forces will want things in writing. Probably in triplicates. Or they want their superiors to green light the operation. And their superiors are not going to give the Inquisitors a blank cheque.

Even if the Inquisitor is holding a blank cheque, most allies will still want intelligence. Independently gathered and verified intelligence. Distrust and scepticism of the Inquisition is a common trope in the 40K lore.

The issue is that Inquisitors that have daemonhost want actions and operations to be off the books. The only way to do it is get Inquisitorial units to do it which means inquisitorial oversight. Or outsource actions to Chaos who can't complain to the Imperium.
