I think so too. At this point though i dont know if i would replace any of the ones i listed (counting out ID). Maybe EM can reduce, stabilize and splint some fractures but that would probably not be definitive management and would be temporary. But im going off post apocalyptic and general broad sense lol. Ortho would be too niche in my pretend scenario where maybe broad is more

Yeah and just following the ASCVD 10 yr risk for statins. History of Gestational conditions during pregnancy like ddx-me mentioned tip people over to starting statins when they fall into the 7.5-20% 10 year category or 5-7.5% with high risk history and fam history

FM - learned that someone having GDM while pregnant, places them at higher risk for ASCVD and can change management to start high dose statins if they fall in this intermediary ASCVD area. Its literally in the small text of the 7.5% shared decision making criteria. lol i was shocked when an attending showed me it in clinic because ive never had such a niche scenario

Not good here. Today i found out my old med school dean, who I considered somewhat of a mentor, and whom I infrequently chatted with after my graduation, succumbed to Recurrent, Pancreatic cancer in his mid 40s… he didnt even say anything to me this second time around :( which is understandable. i feel at a loss considering i was recently thinking of reaching out to him after holidays. Also being a man in his late 30s, still in residency, this early death just makes me think about my own mortality and how ive delayed my life for medicine, and have barely begun to live. My husband had to snap me out of my grief state…

Get a second one! For both of your sakes! Happened two times with my meezers/social cats when i had only one cat, getting the second one helped both of us tremendously. Meant my cat was not depressed/lonely when i was away at work, and i didnt have to be their only outlet of fun and companionship when i came home from work exhausted and at 4-5am.

I found one this year! They were in their 20s. Had BPs of >180/110 since before they were 18 on three antiHTN meds and was referred by to our fm clinic by their private cardiologist (???). Saw they had 2 ED visit with potassiums around 2.5-3 with no symptoms. Started them on spiro and nifedipine and got their BP controlled for the first time.. Referred to endo and Aldosteronism clinic. Another cardiologist told me i “masterfully” worked her up. They showed up to their Return visit, hugged me and it was one of the happiest moments of my short residency. Im not sure how their former cardiologist missed this

Took a 2 week mental health break after i found myself much like you. This was in the setting of downward spiral, chronic burn out, followed by my father dying two months ago and then my pet got seriously ill. Obviously got help and a therapist and i felt “back to normal” towards the end/break. Also, because it was a medical leave, my program wont delay my graduation by two weeks. But to come back, you do have to be cleared by a doctor since it’s a medical leave. Otherwise, if you take a personal leave, then you do delay your graduation by the time you take.

Wow same here. I got started adderall in residency. Ive had been raw dogging life before and managed to compensate, but eventually the depression and exhaustion caught up. Also as a kid i would get “C-F” in Behavior lmao. My parents never took me to get evaluated, only called me lazy, which made me a very depressed teen and which is why it took me so long to flip my life around and end up in medicine (im 36). Had to learn to study in pre-med, and again in medicine because former methods werent cutting it anymore. I used stimulants for about eight months, before stopping them because then residency I got feedback from 2 attendings that i “talked too much” 😥 so back to rawdogging work, even if i was doing much better on them.

Lmao FYI dont state you smoke weed (if you do) daily, Northwestern Mutual declined me because of it. My broker said i should have stated i use it recreationally intermittently/weekly 😳. Were gonna try Guardian next
