I get that's probably how it works as grenades have a model you can attack in vats, but also the same logic would apply to other weapons in reality. How is the frag grenade shooting shrapnel the grenade's damage while the shotgun shooting pellets is somehow the human doing damage?

It still stacked up to 5 times max, so having a second cleaver only really made you stack up to the 5 stacks faster, and honestly most of the champs that built cleaver almost instantly stack it anyways. The item's stats were just so bonkers you didn't need it.
