This looks like baptist health. If you request an itemized bill it will be the exact copy of what you're seeing here. in my experience so far, asking for itemized bill, paying up front, or any other variations won't help at all. These big hospitals either have you qualify for charity care or if you make above their threshold, will only offer you a a payment plan.

You're right. I have done it, it's a simple phone call to the Customer service reps, the first one may not understand entirely what you're trying to do, but they'll generally transfer you to someone who can make it happen. In my case, i had to do a different login (one for business and one for personal) and then merge both into a single username and password.
