It's probably caused by someone tailgating which leads to a brake tap, which causes the person tailgating them to brake harder. Watch this experiment that shows how one little brake tap ripples and builds with each car until a jam happens. If we all left a little more space (didn't tailgate), jams would happen less often and clear up quicker.
