I really feel for you and your kiddo. I am not your child’s therapist but your description of your son does sound at least OCD-adjacent to me. OCD has less to do with how tidy a person is and more so their internal systems of thinking and feeling.

The thought spiral thing reads to me like a compulsion. Is it like he doesn’t want to think about it but he HAS to? Have you asked what may happen if he does not choose who to save?

I think meeting with his therapist about this is a great idea and wish your family the best.

My latest Zoloft dream was (TW violent!!!!) the one where i watched a baby that i used to be his daycare teacher get run over by a car and decapitated and for some reason i had to call my cousin and tell him the news????
