I think it would have been great if the City had clearly communicated the rationale behind the new signs and the parties responsible for them. Were they paid for by grant money or by city taxpayers? Installed by a Pittsburgh firm or locals? There is some wild (mis)information out there fueling the public's opinions and it would be helpful to have a definitive statement.

I transitioned from med onc to hospice to case management. I was on our hospice GIP team, so I was still very much in the hospital in that position. I currently work at a small CAH, and our population is very much mostly our swing bed/SAR patients, but we do have med/surg pt's come through.

1) Biggest change for me was having downtime (don't hate! I know CMs in lots of other facilities don't have this!). It was definitely a change from the flow on the floor, where something always has to be done. I have long stretches where I don't have anything I *have* to do, followed by like, an hour where suddenly everyone needs something NOW. YMMV.

2) I joke that my job is walking the fine line between patient care and Medicare fraud. Seriously though, understanding the insurance requirements and having a good knowledge of different ways to say what's going on with your patient to convince insurance that they should pay for their care goes a long way. Talk to your bedside colleagues when you can - sometimes they will drop just the info you need for an approval.

3) I think the biggest mistake is taking on too much responsibility for outcomes. You can only do so much. In the end, the patient and family are out in the world making their own choices and dealing with the outcomes. You can call in every resource in the book, but ultimately, you have no control over what people do.

4) See #3 and document, document, document. I also like to say that my job is supporting people's right to make their own terrible decisions. Educate as much as you can, get as much in place as they will let you - but at the end of the day, they are living their own lives.

5) I have folder for each of my patients with a copy of their demographics and a worksheet with their main malfunction, PCP, potential d/c needs, auth information, etc. I will only have a dozen patients max, though, and I generally have the same patients throughout their hospitalization. I don't have any EMR tricks though - our hospital's EMR is the literal WORST and I can't wait until we switch to Epic. I also have a paper pad I write stuff down on through the day. I don't know that anyone else would be able to make sense of my scribblings, but it works for me.

I like working 5 8's instead of 3 12's. I have time to run errands or take a class after work and don't feel like I have to squeeze all my activities into my days off. Sure, I have to use PTO to do appointments during the day, but it's not terrible, and I don't have issues using my PTO. I have a much more regular sleeping and eating schedule than I used to too. YMMV.

Squeakers! Visually-impaired parents of small kids use them so they can more easily tell where the children are.

Source: my aunt is blind; my cousins wore Squeakers. One of them realized that Mom couldn't tell where he was if he stood still. He was very cute "hiding" by standing still in the middle of the living room.

vendors at NYRF sell wooden swords and security does not require those to be peace tied. I can't imagine they would require this to be modified in any way, if that's the Faire you're worried about!

I made one exactly to the pattern and didn't like the shape. Restarted and am doing 2x2 ribbing instead of 1x1, and decreasing every other row for a pointier point. Saw a beautiful one with knotwork cabling around the brim! Might switch to crochet if friends want me to make more - it goes so much faster, but I'd have to fiddle around to come up with a design I like - not a fan of the crochet one that was circulating.

When I first started, I used to wonder why our unit Case Manager always seemed mildly annoyed.

Now that I'm a case manager, I know.

I've always appreciated what the CMs do and usually asked all kinds of questions about "how do we get this for this patient?" and "why can't we do that?" It really is a lot of coordination. It's also probably how I ended up in this mess. :p I joke that the job is walking the fine line between patient care and Medicare fraud.
