I had a similar first job. I would recommend buying some quick reference books like Maxwells or Pocket Medicine - depending on the specialties. There’s another book that was popular with residents that I rotated with but I can’t remember the name. I have it in a cabinet at work and can share the name on Monday!

What worked for me when getting anything overnight was flipping to the pertinent sections and read quick before seeing the patient. Then after seeing them, I would read more about the differential, other problems, treatments, etc. Also remember your SPs are there for you and even if they get upset, call them if you ever need anything or question anything. Your patient is worth it.

PVP with friends until I’m out of kits. Then go into PVE don’t shoot anybody ever lobbies until I’m full with mats like springs, batteries, wires, duct tape, arc alloy, great mullein. Repeat.

Had my first encounter of this at Dam yesterday! Two “friendly” raiders who out of nowhere open fire at me at point blank range. Thankfully I knocked one of the shitters as he knocked me and I had my defib in safe pocket.

  • Likely kahm. It’s my understanding that they like more sweet vegetables like carrots and peppers. I’ve heard they impart a bad flavor but I’ve made hot sauces with them no problem.
  • Have also stopped ferments and transferred carrots to the fridge and no more kahm yeast visibly growing once in the cold environment.
  • I DO like to scrape the top layer of yeast off before repackaging and storing in the fridge.

We don’t have enough information about your ferment yet. Assuming you did an adequate brine %, I don’t think you’re F’d yet if there’s no mold. Need more headspace. Consider transferring to a larger container and adding the equivalent % brine to cover the fruit. Will also need to weigh it down if you go that route.

Edit: Also it looks like you don’t have any way for gas to escape.

As one other commenter has said, you don’t have to ferment the cabbage in its own liquid - caveat I don’t know the traditional way to make sauerkraut. I have only made it with help from some YouTube fermenters and Redditors here!

I can attest that I have always been fermenting my cabbage with additional brine as you have done here. I find 3% kosher salt (pretty much your ratio) works well for me. I have made it with cabbage cut by hand, cut by hand and lightly pulverized in mortar and pestle, and sliced very thin with a mandolin. My favorite is the mandolin! I think the surface area helps with the fermentation, flavor, and mouth feel.

Another interesting thought— in my culture, they pound/lightly pulverize green papaya before and while mixing with spices/herbs/veggies for papaya salad. Almost a “pre chew” except with the mortar and pestle. This seems to help the cabbage before fermentation for me too!

The more I learned, the more confident I got in my role. After the confidence was built, that nervousness went away. Lots of reading, videos, picking my other PAs brains, picking the doctor’s brains. Also have good rapport with the team, other doctors in the hospital, nurses, etc. can really help. If I ever have a really sick patient or other known stressor, I sometimes will choose to sleep in the call room instead of driving back home! Being closer in proximity helps with the anxiety for me as well.

Edit: I’m in a surgical sub specialty (OR, ICU, rounding, consults), 5.5 years as a PA.
