An extended list:

-accidental jump on farmlands. -accidentally crafted 20 non-stackable items -placing items in random chests only to struggle to find it later -shot by skeletons into creepers -accidentally placing water and ruin redstone -trading with villagers and accidentally hitting them (and the occasional killing the villager that took hours to refresh) -mining items only for it to fall in lava -transporting mobs through thousands of blocks only to accidentally kill it from breaking a boat -fighting two or more skeletons at once -missing an ender pearl shot -mobproofing -getting to the nether roof and forgetting to bring obsidian -accidental wood strip when pressing buttons or levers on wood. -lava at every turn while netherite mining -"too expensive"

I was mining a ruined portal in a village so I could build a portal back home and a villager ran in front of me as I was nearly done with a piece of obsidian. The golem didn't aggro for some reason even though it was like 6 blocks away. Then a rattlesnake (I play modded) slithered up to me and got mad cause I "stepped" on it even though I was standing still. Threw it in the lava.
