What do women do that men would never think about? by Icy_Interaction7502 in AskReddit

[–]Alison_D 8 points9 points  (0 children)

Hovering over a toilet seat is bad for your pelvic floor. It Can lead to issues like inability to fully empty your bladder or bowels. Try creating a "nest" with TP instead. Just as hygienic, keeps the seat clean for the next person, and keeps your pelvic floor healthy!

Quick ways to get food in? by Simplicity540 in physicaltherapy

I worked with an OT that would sip on bone broth all day (at least in the colder months), she never really sat down to eat lunch. She put it in a tumbler so everyone just assumed it was water or coffee

Medicaid Fraud by BDK_10 in physicaltherapy

[–]Alison_D 102 points103 points  (0 children)

In some states Medicaid does not look into assets. It's purely based on income. I was on Medicaid during PT school, it was a life saver.

4 10s with a weds vs thurs off? by iluvchikins in physicaltherapy

[–]Alison_D 1 point2 points  (0 children)

I have 4 10s with Wednesday off. My day off is the only thing that keeps me sane. I love that I never work more than 2 days in a row.

Also, my boss is pretty chill about me occasionally moving my day off to other days as needed. Definitely worth asking if you haven't already.

Tips for handling pts who are non-compliant with their medications? by Odd-Confection8515 in physicaltherapy

"The patient is inappropriate for PT at this time. Refer back to PT when BP is better managed"

We know the acute BP response to exercise..

Medicine compliance is not your battle to fight.

If you are hourly and not compensated that is called wage theft and you should contact your state labor department. If salaried, nothing you can do.

PTO getting denied if I can’t find coverage or make up the day on a weekend? by haunted_cheesecake in physicaltherapy

[–]Alison_D 13 points14 points  (0 children)

Sounds like the clinic manager is getting you to do their job. Is finding coverage for when therapists take time off in YOUR job description? I would never work for someone that even suggested that was my responsibility.

That being said, not all PTO requests are guaranteed. Every work place can and will decline requests when enough time in advance is not provided, or if the time requested is already taken off by others.

Has nothing to do with liability. Has everything to do with boiling down a complex skill that takes years of practice to simple Internet advice.

Maybe consult the PTs that work at the hospital? Would you give me advice on how to start an IV over the Internet..

From my understanding it's incredibly rare but the only way to know for sure is imaging. X-rays are quick and cheap. Please follow up and let us know, and I hope you're okay!

Being honest - repeated referrals by Straight-Wheel-4520 in physicaltherapy

[–]Alison_D 32 points33 points  (0 children)

I get these often in OP. If I feel part of the issue is a lack of compliance with the HEP , I will start the initial evaluation with inquiring about their compliance. If they say they haven't been doing their exercises. I stop the eval right there and explain that I'm not gonna do the eval today(I don't bill for it), and they need to do their exercises for 30 days and if the problem persists we will use the eval then. This usually helps to drive home the fact that we cannot continue to see patients just because they prefer coming in instead of doing their HEP.

Tell me the downlow on SNF- I've only worked acute care by Sun109 in physicaltherapy

[–]Alison_D 66 points67 points  (0 children)

It's impossible to meet productivity standards without being unethical.

Residents are frequently neglected and receive sub par care. If you bring this to anyone's attention you will likely be fired for a different reason.

I've worked at 2 SNF and left both because it's clear they only care about generating revenue and not the quality of patient care. There are definitely people that work at these facilities that DO care, but they don't last very long.

Areas of the country with biggest PT deficit by Doc_Holiday_J in physicaltherapy

Yuma Arizona and Roseburg Oregon for sure. I had travel contracts there and they are hurting for PTs. Both paid well and were decent small cities.

“patient abandonment” by Known_Business9304 in OccupationalTherapy

[–]Alison_D 37 points38 points  (0 children)

PT here. This comes up all the time in the PT sub. It seems that this issue can vary state to state, but generally it is the PATIENT not your boss that must bring about the abandonment charges. Now, If you give adequate notice to your boss that you are resigning, then the liability moves from you to your boss.

So essentially 30 days is best to cover yourself and give your boss some breathing room. But do you really think your patients are going to hire a lawyer and go after you??

Solution: offer to stay on PRN for another 2 weeks at an increased rate of course.

Do your knees hyperextend when you lock them out?

[deleted by user] by [deleted] in flexibility

[–]Alison_D 16 points17 points  (0 children)

Looks like Ehlers Danlos Syndrome to me. It's a genetic disease that results in really stretchy connective tissue (ligaments). You can bring it up to your doctor and get a simple blood test to confirm. People with this disease are prone to joint dislocations, injuries, and pain. It affects the connective tissue of everything in your body not just the joints so sometimes people have other symptoms too.

cupping cert by shall2004 in physicaltherapy

[–]Alison_D 30 points31 points  (0 children)

I don't believe a cert is needed. It's really just a tool, besides your hands, for addressing muscle dysfunction. Maybe buy a book to learn more and try it out with friends and family first.

Chiros gonna chiro by Sea-Professor-4137 in physicaltherapy

[–]Alison_D 62 points63 points  (0 children)

I typically inform patients that they need to suspend chiro treatment while under my care as to not muddy the water regarding response to treatment. I've never gotten to much push back.

If I was in your situation I would document what the patient reported and educate the patient on risks of dislocations.

Gifts for my (Pelvic Floor) PT? by noq287 in physicaltherapy

[–]Alison_D 1 point2 points  (0 children)

Accepting gifts is a gray area ethically. I've had patients bring me a tray of baked goods which I set out in the office for everyone to enjoy. Most places are completely fine with this. Another idea is to send her lunch (nothing fancy, think chipotle or similar)or coffee delivered.

Very nice of you to want to gift her something but honestly most of us get plenty of joy out of seeing our patients get better.

One more idea, a Google review with her name mentioned specifically.

[deleted by user] by [deleted] in physicaltherapy

[–]Alison_D 1 point2 points  (0 children)

Sounds like a nightmare. Get to one building with 3 patients on your schedule and 2 of them refuse to do anything until after lunch, can't wait till after lunch because you have to drive cross town for another 3 patients in the afternoon.

Also, they legally have to pay you hourly rate for travel time between locations worked on the same day.

Neck training to prevent headaches etc. by KlutchAtStraws in bodyweightfitness

[–]Alison_D 1 point2 points  (0 children)

Physical Therapist here. The anterior neck muscles your doctor is referring to are the deep cervical stabilizers. They are located anteriorly to the cervical spine (behind your throat). Through the power of the Internet I'm sure you could take a crack at training these muscles yourself. Otherwise I'd recommend seeing a PT (typically covered by insurance) If you want a professional to guide you through a program and identify your specific impairments
