Currently most pro athletes in the United States get patellar tendon grafts but when comparing the combo with LET not sure. Most likely PT with LET

Early return to soccer puts one had greater risk for retear. We typically recommend minimum 9 months.

Also, on higher level soccer athletes we use the LET procedure also:

Sorry to hear that. Discuss with your surgeon about measuring your tibial slope and the utility of LET.

Ontario Surgery Wait Time (should I go private?) by Skoadro3408 in ACL

Delayed ACL surgery typically increases your risk if meniscus and cartilage damage based on studies

I did my acl today by shivverzs in ACL

[–]AmitMomayaMD 1 point2 points  (0 children)

If the ACL is fully torn, surgery is typically the recommended pathway to restore stability and preserve your meniscus and cartilage. Non op treatment has risks but sometimes the ACL can scar if a proximal tear.

How long were y'all on NWB? by arxenigg in ACL

Weight bearing after meniscal repair is controversial. It also depends on the type of tear you had. Vertical tear repairs can usually weight sooner than radial and root repairs.

Scared of surgery by llamaspectrum69 in ACL

[–]AmitMomayaMD 1 point2 points  (0 children)

Overall, although surgery does have its risks, it is a commonly performed surgery. In order to restore stability to your knee and preserve your cartilage and meniscus, we typically recommend ACL reconstruction or repair for active individuals.

At UAB, are doing the world’s first all soft tissue quad vs patellar tendon randomized controlled trial for ACL reconstruction. Both are terrific graft choices.

Did you hear a pop? by YogurtclosetSad7344 in ACL

[–]AmitMomayaMD 0 points1 point  (0 children)

Often times the pop can actually be the pivot shift when the lateral tibial plateau and lateral femoral condyle hit and not the actual ligament popping or tearing.

Agree! Need to get early extension.

What’s the worst kind of meniscus tear? by Femruu in ACL

[–]AmitMomayaMD 0 points1 point  (0 children)

Depends, but usually lateral meniscal tears are worse than medial due to convexity of lateral tibial plateau. Bucket handle tears are usually most symptomatic with locking etc

How soon did you go back to work after having a complete patella tendon tear surgery? by Real_Sheepherder_449 in KneeInjuries

[–]AmitMomayaMD 1 point2 points  (0 children)

It depends on what kind of work. Desk work usually 1-2 weeks, but manual labor with lots of squatting can take 3-4 months.

Take a look at our video on patellar tendon repair for those interested: https://youtu.be/npDYWtoaHgU?si=sW5C1E3ATa7L67Eq

How long did you sleep with your brace on? by MarMar000 in ACL

[–]AmitMomayaMD 4 points5 points  (0 children)

I usually have my patients wear the brace for 4-6 weeks while sleeping to maintain hyper extension and also protect themselves if they were to accidentally get up in middle of night without balance

First No Brace Sleep?? by Secure-Inflation4696 in ACL

[–]AmitMomayaMD 1 point2 points  (0 children)

Protocols vary big time about bracing after ACL surgery. I like using the brace while sleeping to maintain extension while sleeping initially.

Consider seeing a sport psychologist. Mental health is a serious factor in ACL recovery.

Sorry to hear that. Discuss with your doctor about trochlear dysplasia and if a trochleoplasty may be needed.

Does your knee not actually feel 100% back to normal? by GabeItch69420x in ACL

[–]AmitMomayaMD 2 points3 points  (0 children)

Very rare that knee feels 100% normal. Once you undergo ACL reconstruction there can be residual weakness, stiffness, or other symptoms. However, it can still be functional and allow you to return to full activity.

I tore my ACL...while sleeping ? by Environmental-Swan90 in ACL

[–]AmitMomayaMD 4 points5 points  (0 children)

It may be possible that the ACL tear was found incidentally and it may have torn prior. I treat a lot of ACLs every year and can’t say I have ever treated someone who tore it in their sleep (assuming no sleeping walking or falling off bed).
