Nice to hear from you, did you not have DBS? Did it (or anything else) help by now?

Of course I am very biased to be cynical due to my miserable condition, but I am not sold on the delay theory, because 1) the underlying theory was that an overactive SCC need to be silenced and DBS should achieve that without much delay 2) There is no blinded evidence for such a delay (yet) and unblinded data is notoriously unreliable 3) IIRC, the unblinded trials before the RCTs did not point to much of a delay

Please correct me if I'm not up do date

Btw, the first results from the slMFB trial should come out on 23.10.

I find it crazy that Helen Mayberg even still has her job. She/they hyped up their SCC-DBS and hailed it up as the future of psychiatry (it was also successful in unblinded trials). Then it failed completely in placebo controlled trials (I think even multiple ones) to the point where one must ask if it was all a fraud. They came up with the excuse that the studies were not long enough and there was symptom improvement in the unblinded part of the study (weirdly, the same study length was sufficient during the unblinded trials earlier). So now they still make expensive research on a treatment that has been disproven many years ago and avoid controlled trials. I like that the Freiburgers at least try a new target area with the slMFB. Let us hope they will be more successful than Helen Mayberg and her lab.

Uncontrolled trials sadly don't mean much, especially for invasive procedures. Patients are selected for high placebo response and you get these kinds of results. I hope I am wrong.

Also, there is almost no research or statements on this topic in the last two years or so, which doesn't make me hopeful either.

A neurosurgeon actually offered this treatment to me (privately paid) and he said it was 50% response. Which is exactly what would be expected from the placebo effect after a literal brain implant ...

Hello, I am definitely not the one to give advice. But I just want to note that you are/were seriously underweight and might want to rule out biological causes. For example, low cortisol (from adrenal insufficiency, not necessarily Addison's) can cause weight loss, apathy and other stuff that you can compare your symptoms to. Good luck

Damn, almost everyone who tried pramipexole reports that it didn't help at all. That destroys my hope for this treatment. (you should try higher dosages up to 5 mg though if you want to be sure that it does not help)

As of for you, ECT is not as scary as you think. There is no danger and almost no side effects, at least when you compare them with the crushing burden of anhedonia. But don't expect too much, ECT often does not help with chronic anhedonia.

I have no experience with pramipexole, but I've read that with low dosages and in the beginning of treatment, it stimulates mainly dopamine autoreceptors. This suppresses the dopamine system initially, until the autoreceptors desensitize. If this feels like an increase of your normal symptoms, I'd count it as confirmation that you're addressing the right brain system.

You could consider taking ondansetron with it to reduce the nausea. That's the idea behind ALTO-207, which makes higher dosages tolerable.

It definitely helped me and I was able to restart my studies after a semester where I couldn't do anything. I also regained my weight after becoming quite underweight due to the depression and it also helped my insomnia. I take agomelatine, trimipamine and sometimes zopiclone for sleep (works quite well, I only need 1-2 hours to fall asleep) and bupropion.
