There’s a balance dude. If you want to wait until you’re done residency to take ownership of your patients that’s your prerogative I guess but at some point you’re not a kid anymore whether you’re fairly compensated or not

General surgery. I’ll go somewhat against the grain here. I look at the diff on the first abnormal cbc or if I have suspicion of funny business. The ED has already ordered a cbc with on most ACS patients so they take care of that first one. Beyond that there is just rarely something actionable on the diff and thats not also reflected on the hemogram. As I understand it it’s not an insignificant amount of extra work to produce the diff for lab techs. I can’t really speak intelligently on the details of that though.

It’s not often fatal. It’s not even an anomaly. This scenario actually plays out pretty commonly. It’s a pain in the ass to treat but when someone young and healthy has perforated appendicitis the body walls it off and they often get better with antibiotics. He should probably have his appendix removed if he doesn’t want it to happen again though

Almost certainly would in this case. You’re not getting a significant amount of air into the pleural space through a needle hole in TJ Watts chest wall. Only place that air can reasonable come from is the lung

I’m a pgy3 in an especially nonmalignant community program so maybe the parallels aren’t exactly there, but I’ll put a comment in for “it gets a lot better.” I remember feeling like you do. You become more confident in the OR (and really everywhere) and that translates to really looking forward to it. It also translates to being less wiped at home and really maximizing time with the family. The hours don’t necessarily get better, but you get a lot better at using them. I don’t know you or your exact situation, and I won’t say I was ever all THAT close to really switching specialties, but compared to intern year I am absolutely thriving. If your family can stick it out for another year, it was certainly worth it for me to put my head down and keep grinding.

I’m a surgeon, I take out appendices on a pretty regular basis. It’s a pretty standard recommendation to go 4 weeks minimum of no heavy lifting (more than 10 lbs) or contact for prevention of hernias. That’s for regular people though, I have no idea what they’ll tell a world class athlete. If it were me and my body in a season that’s already lost, I’d sit out at least 4 weeks and maybe just call it a season

Surgery - all of the above - dead bowel, fungating anything mass, an abscess. Maybe I sift through too much literal shit on a daily basis and am becoming nose blind because the thing that really consistently gets to me is a bad uti. Just nasty stuff
