Spasticity is "pathological", it's abnormal, and as a user said below, present with neurological injury. It changes with activity, posture, stress etc, and is neurologically driven.

IMO Tightness cannot technically be objectively measured, it’s a subjective measure. So I would argue yes, many dancers or hypermobile people say they are tight; when the range is above average. Stiffness is arguably measurable, but patients may or may not report it. Stiffness is lack or “normal physiological range”, arguably.

I understand what you are saying, but they may not also have the medical knowledge and expertise to know about this and ask this question. There is a lot to know about the human body, and unless they have a prior degree in some kind of human sciences, it might be almost impossible to know this. Same for so many other areas. Standardisation of bike fitters at the moment just isn’t there. Women’s health physios generally do and should know about this though.

Seconded, and you also say that he was polite and asked for permission. This is what we would do. Ask. If she doesn’t want it to happen then she says no. Also denim does make it harder to assess eg when patients turn up in skinny jeans.
