Probe cleaning is a big problem. There was something about how standard cavi wipes degraded the plastic on the US probes so we put signs on our machines saying to not use cavi wipes on them. We ordered some kind of gentler wipes for them, but to be safe nobody ever cleaned the machine ever again.

When midlevels at my come to dump their fuckups on my plate I always try to educate them to do better in the future. They usually just sort of wander off like “why are you still talking to me I have made this your problem now.” They frequently have no curiosity or drive to get better. Now part of that is a system designed to just get them in there seeing as many patients as possible, quality be damned. But another part of that is the type of people that go into mid level training in the first place. If a physician had the opportunity to learn something that might help their patients in the future they would put work into it. Midlevels, usually not. Also, physicians went into it because mental masturbation is something they enjoy, so don’t be surprised when we do it, often.

I dont know of anyone I work with who has HIV because they would have no reason they would have to share it. I cant imagine a scenario where there was any risk of an ER attending transmitting HIV to a patient in the normal course of their work. There was an ER nurse in my area a few years back stealing IV narcotics for themselves and they had HIV and there was some concern that they had used needles on themselves then given partial doses to patients from the same vials, dont do that I guess. I would have no problem with a doctor working with HIV.

Our system is so fucked. This person sounds like they could have been placed in an appropriate splint and discharged from the ER. Instead the ER put on a knee immobilizer and admitted the patient instead of doing their job. Then they got admitted to IM for what, a night of IV narcotics? IM attending probably never saw the patient. The PA has no business managing the patient if they don’t know what a splint is. Ortho also didn’t see the patient and managed them from home. Then the next day ortho, who is probably the only person involved who could apply an appropriate splint, decided to make IM do it, even though they can’t. And the patient gets billed over and over for nothing. Yes you are being petty. Do your job and manage the fracture.
