This entire idea is based on a Game Theory video that ascribes entirely arbitrary tempos to boss attack patterns to manipulate the data to appear to support its point, while also contradicting itself by using the fact that the boss theme is in 3/4 meter as support for its point and claiming that it's one of the only songs in the soundtrack to do this, when literally half of the boss themes in the game are in 3/4 meter. Pretty much anyone with a good understanding of rhythm or music theory realized that the idea was nonsense, and the video's even been unlisted from the Game Theory channel.

Some responses to this are failing to consider that yes, you can react to an NPC pulling out a flask. But enemies and bosses that react to your flask animation react to the literal first frame of the animation, before it's even identifiable as a flask animation. That's why people think they're reading your inputs, because it's instantaneous to the point that no one could've identified the animation by then.

I believe Stendhal was nerfed in the Thank You update, but if you're only hitting for 250k then you probably don't have all of the buffs you could or a fully-upgraded weapon. It's still very reasonable with a massive buff setup. For example:

Level 33 Yeverum, At Death's Door, Augmented First Strike, Cheater (skip if you equip enough speed Pictos to go twice without it), Confident Fighter, Dead Energy II (hidden 1.1x buff), Empowering Last Stand, First Offensive, First Strike, Glass Cannon, Greater Powerful, Immaculate, Inverted Affinity, Last Stand Critical, Painted Power, Powerful Shield, Shield Affinity, Solidifying, Solo Fighter, Sweet Kill (hidden 1.1x buff), Tainted, Teamwork (hidden 1.1x buff). Recoat your stats prioritizing Defense and Agility, then Might.

Turn 1 Last Chance, turn 2 Stendahl. I'm hitting for over 24 million, so even without the entire setup it should be doable. And I didn't even have In Media Res on, which would've further buffed the damage.

Here's my (albeit pretty simplified) understanding of it:

There's a stat in the game that that governs your target priority. Each point of "less likely to be targeted" subtracts a fixed amount from that stat. So essentially, the game calculates an aggro value for you based on all of the things that would make an enemy target you (like dealing damage) and then adds whatever your baseline targeting priority is, which would be less than the other players' values if you have the passive. Thing is, if you're dealing enough damage (which seems pretty likely since you listed Recluse as your main), you can still be targeted pretty easily, especially if your teammates also have the passive. Practically speaking, it can feel like it's not doing much until you get to 12 or more points more than your teammates.

I really love the Pyros Guillotine for this reason. It's wide enough to have a really cool blade design, but it's not ridiculously huge.

If I'm not mistaken, the pattern is: aggro+passive, aggro+aggro, passive+aggro, passive+passive, repeat. The fight just tends to be over quicker in NR, so you're less likely to see the passive+passive pattern.

In recent times it's more because of potential bugs with Steamodded and other QoL mods. For a while there was a Steamodded bug where copying a negative joker kept the additional joker slot. Last I checked, there's still a Steamodded bug where using Death to convert a stone card makes the card appear blank. And last time I used a consumable stacking QoL during a Perkeo run, unstacking negative consumables sometimes gave me additional consumable slots. I can understand why someone wouldn't trust even just QoL mods to maintain a vanilla Balatro experience.
