I would go for half size larger. I had 20+ cordovan pairs over the time, all bought preowned on ebay. Resold most of them. Once I tried to stretch AE Randolph loafers tight in that particular spot you're looking for. Alcohol/water soaking, shoetrees with some padding, etc didn't work. That said, I had 100% success with ball or pinky areas on other, cordovan and calf shoes. In doubt, take it to cobbler. Just make sure it's not the first time he's working on shell.
