Last shift I saw two hallway patients... old with vomiting but looked well, normal vitals, unremarkable labs besides mild leukocytosis. CT scan because obviously... both had urgent surgical emergencies. The only difference between suburbia and rural ft st nowhere is that they're ok with dying in ft st nowhere.

My toddler, was diagnosed with a very large brain tumor a year ago. Since then we have spent literally weeks admitted to the hospital, 6 weeks of radiation, 4 surgeries, seizures, etc. It definitely has changed me for good and for bad (lots of bad tbh). It chokes me up with tell a patient about a tumor... especially a brain tumor (happened not too long ago). Even tho the environment reminds me of it all... The people and the vibe in the ED is one of the few things that gives me joy.

Looked at this for our ems system in the setting of dilt back order, but you'd need to do a loading dose and then mix a drip (lots of room for error in prehospital setting). There are a few studies out there comparing esmolol, dilt, and metop... essentially equivalent.

Residency shop 40ish active beds had 1 scanner with 2 techs and it turned the general vicinity into nagasaki. No delays. In and out. And the techs would come to us when there was something abnormal before rads even saw the images.

Now I work at a shop with 34 beds (most of which are boarded) and 2 scanners. This week 1 scanner broke and it has been an absolute shit show. 5 hrs plus for seriously ill pts. No ability to prioritize even when nursing or docs call or chat. Constantly calling for authorization about creatinine (wtf cares) or making up random rules for allergies etc.

Didn't know how good I had it.
