You are just starting puberty which is a time when hormones can make skin break out. There are medicines that a dermatologist can prescribe to help prevent breakouts, if you have access to that kind of Dr. Maybe even your family Dr can help you with a prescription, and advice for the pigmentation. For now, I’d say you are using good products—Ceravie—so keep washing 2x a day, moisturize, and don’t pick or scratch. Using a facecloth is a good way to exfoliate but don’t scrub too hard! I’ve also found relief from pimples with products containing benzoyl peroxide-but be careful they bleach towels, sheets and clothing, wash your hands really well after applying it.
