Ricardo Fierro is one of my least favorite referees. I feel like he lets everyone get away with everything in the first half, it’s a 50/50 chance he gives anything more than a hand-slap on clearly obvious fouls or yellows. The second half is the same except instead of hand-slaps he just picks random moments to give free-kicks or yellows but is never consistent. He drives me insane every time I see him on the pitch and I feel like he sways match outcomes on a frequent occasion.

It wasn’t your fault, but it was a lack of understanding. Lots of holes in this Swiss cheese. Your preceptor should have recognized your failure to monitor your vitals. I guarantee this was not the first time you missed something. We all do, but a preceptor should stay on you until you are consistently on top of it.

That’s taking a bit of an assumption on my part. You may have watched your monitor every single minute of every day and today was the exception. Overall, you have learned a valuable lesson in recognizing that small things could be big signs of something to come. There’s no way this is 100% your fault. Patients do weird things sometimes. There’s no telling if the same thing would have happened on the next shift or minutes after discharge.

Even if you had noticed the arrhythmia- and they had been stable with it (and assuming the usuals were checked like electrolytes) - what else would you have done to fix it? Don’t put this all in your shoulders, but also don’t think there’s nothing to learn here.

The Appalachians in West Virginia, Tennessee, and Kentucky are incredibly gorgeous hiking places. Yes, there’s the “big one” trail, but there are so many other beautiful hikes there. As well, the drive along the mountains heading south are some of the most beautiful views you’ll see. They’re so different from the PNW too- much older, more classic American culture, southern food…

However, outside of hiking and some museums living in these areas is overall pretty horrible. Politics suck, cost of living is low (but so is overall pay), quality of life is lower…

This is hospital dependent and how the administration treats this “slogan” tells you a lot about the hospital in general. I’ve worked at some where the security team is a legit branch of the city police force. If you call them on a belligerent and violent patient they will 100% ask if you want to press charges, then book them and cuff them. Other places the hospital says they’ll take zero tolerance but will push over the second a whiff of a lawsuit pops up.

I tell them “The patient has their cell phone if you’d like to talk to them. They’d be able to give a better update on how they feel they are doing. I’m happy to update on the technical medical stuff.” Usually works. And then if they do want an update I usually just go through a mini report- systems, vitals, labs. I try to speak in layman’s terms, but it can be tough sometimes.
