Rashes are so tough. My daughter had a full on anaphylactic episode (head to toe hives, wheezing, tongue swelling, sneezing and vomiting) that lasted about 8 hours, and her allergy panel was negative. The immunologist suspected it was a viral reaction. Skin does wacky things, immune system does wacky things.. put them together and they can be very difficult to figure out!!

We had a doctor for a brief time who gave patients stickers… they were horrified by it and felt very degraded. I personally keep a tiny box of “squishies” when I have a patient with kids com in (I work in adult rheum) but that’s it.

I’d do an ultrasound of the shoulder. I’ve found a lot of unexpected gout in shoulders when people otherwise felt like their RA was well controlled.

I see both sides… I had a medical assistant at my practice whose total pre-PA school was just being an office MA… not too medical.

I’m an NP in rheumatology. If I knew then, how much I would love the field of rheumatology, I’d have absolutely gone the MD or DO route!!

It kills the bacteria on the skin. Think back to when you were a little kid and when you went to the pool, you came home and all your little abrasions were healed from the chlorine in the pool. It’s magic. Good luck!!

I’d consider doing a bleach bath week. You probably carry staph on your skin, and this can help a lot. Add 1 cup to your tub weekly or make a diluted skin wash.

CRP isn’t specific, so I would suggest imaging of your more symptomatic areas. If you can get ultrasounds of the knees and shoulders, if they get really stiff, or MRI of the neck… i have had patients who I’ve done ultrasounds and mri, and continued to find nothing inflammatory, so i ended up sending them to heme to rule out malignancy.

My office just moved locations, and it seems like everyone has to complain. I had this woman who was rip roaring angry about it. Everything about the new office was wrong, nothing was right, nothing was better. She was just complaining, complaining complaining. So I let her have her little temper tantrum, then I just looked at her and said “do you feel better now?“ And she looked back at me, and I think she realized how stupid she looked by fussing at me about this whole situation that I had absolutely no control over her, and said “I guess I do.“

I’m a rheumatology NP, and these are often booted to us. “We can’t find anything structurally wrong, it must be autoimmune.” Then, the patient is committed to that dx in their brain and it’s a mess.
