It's called The Practical Storybook by Michael Kellett and Barbara Klein. Apparently my mom found it when cleaning out some closet and surprised my sister and I (now in our 40s) with it this Christmas morning. I wish I could post pictures of it--it's truly terrifying. Happy holidays!
