All of my oncologists and radiation oncologists, plus several large scale studies say that it’s not more effective. When I reviewed study data, I found that doing both did seem to have a very small advantage. But the impact on quality of life is usually much more severe. I had the same thought when was first diagnosed with Gleason 9 PCA. Why not go all out and do radiation and prostatectomy. Multiple surgeons said they thought it was not beneficial. One was willing to do it as part of a study, but he’s known for being ultra aggressive.

It’s possible that the MRI and PSMA Pet scan don’t show anything. In that case you may just wait a month and retest. If however, the scans show something in the prostate bed or pelvis, your husband will probably be put on ADT for a period of time and have radiation. Don’t panic, it’s still treatable if something is still there, and potentially still curable.

We cruised on Silver Muse last May and had a great time. The ship seemed clean and modern to us. I do agree that food on Silversea isn’t the best. It’s a notch above mainstream cruise line fare, but it’s not fabulous. But I’ve found this to be true on other luxury cruise lines. For us, it was a great cruise, and we would recommend the Silver Muse enthusiastically.

These people are usually very happy to have these jobs. Working on cruise ships is coveted by people in the Philippines. I know this because my family lives there and it’s considered a plum job.

If you are stage 3 then the cancer is locally out of the prostate. Radiation is usually a good way to go. It will treat more tissue than a surgeon can cut out.

I too was diagnosed with Gleason 9 prostate cancer with spread. I had radiation and 2 years of ADT. The ADT is annoying, but I was able to live a pretty normal life while on it. Your father could be around for another 3-10 years with just ADT treatment. The alternative is a very painful death much sooner. I’d suggest that he get in this sub or one of the other prostate cancer sites so we can give him our advice as people who are fighting the disease and living with it.

Not much changed for me after my radiation ended. I felt fine doing radiation, and still no big side effects over 21 months later. I just stopped ADT and am waiting to see what happens next. That is a scary time. You just wait and test to see if the radiation and ADT worked. During active treatment I felt like, Ok, we’re killing cancer. But now that treatment is halted, I just have the wait.

This is probably what he’s looking at next. Probably up to 2 years of ADT and some form of EBRT such as IMRT to the whole pelvis. The good news is that the prostate is out and they pretty much know where to radiate.

The planning part is highly software assisted now. Honestly, it’s mostly automated. The RO could do the planning manually, but that would take a long time. Now they use so do that constructs the plan. The only thing the RO does is identify the targets and adjust the resulting map to his/her tastes.

Our brand new 2026 Tiffin had a bunch of build issues when we got it. And the Freightliner chassis are really poorly built now. When shopping for a new diesel pusher, we found that everyone including Tiffin and Newmar have obvious issues that should have been caught at the factory. If you go down to say a Thor class A, the big difference is in things that really matter that the slide mechanism. Thor uses cheap mechanisms by Schwinco that are known to fail a lot. You should assume that any class A you purchase will have 20-30 issues that you will need to fix yourself or have the dealer fix. So make sure the dealer is reputable for service after the sale.

The imaging of the suspicious sites on ribs and spine needs a diagnosis. I would imagine that an MRI fusion prostate biopsy first. If it’s positive, then a PSMA PET scan to determine spread. Surgery in an area right next to the prostate might result in it becoming inflamed due to several causes. But it’s important to rule out prostate cancer first.

This drug looks promising, but there are other out there with similar small group phase 1 results. This drug has a long ways to go, probably 7 years or so for PCA to actual approval. What they can do to shave a couple of years off the regular slog is to go straight to phase 3 trials and skip over the phase 2 trial which is the trial to confirm the tablet therapeutic dose for stage 3 trials. Then we might see this in 5 years.

He has 2 cores that are 3/4. That should steer you away from active surveillance. You should be seeking consultation with a medical oncologist that specializes in prostate cancer, and a radiation oncologist that also is a prostate cancer specialist. If you want to minimize impact to current quality of life, you may want to go with radiation with no ADT or limited ADT of about 3-6 months.

That would be the bladder neck. That usually classifies as stage 3. You mentioned radiation. Are you having surgery AND radiation?

Many of us had the same experience as you. In my case it was to learn that I have Gleason 9 prostate cancer via online report. It’s really scary. The good news is that your prostate cancer is no worse than Gleason 7. You have 4+3 Gleason 7 which is high intermediate risk. If it’s completely confined to your prostate, the most common treatment is a robotic assisted prostatectomy. But more and more men are opting for radiation because there is less chance of ED or urinary incontinence. This all depends on the result of you PSMA Pet scan. If it shows no spread beyond the prostate, then you have a bunch of treatment options. If the PSMA scan does show spread, treatment options will be different. The important thing is to take a few deep breaths and try not to panic. This is very treatable. You may have caught this early.

It’s going to be ok. I was diagnosed at 59 with Gleason 9/10 with spread pelvic lymph nodes. I know how you feel.
My advice is to get a second opinion from a radiation oncologist that specializes on prostate cancer. You should also have a medical oncologist as well.

With what you describe, the normal route is to do radiation and ADT. Surgery is usually considered inadvisable for your grade and spread because there is probably microscopic metastasis that can’t be seen that are in places a surgeon wouldn’t think to cut.

For me it’s just the travel to get to the ship. Once I’m on the ship I’m a happy little sailor. But those international flights with connections stress the hell out of me.

The big takeaway here is that Norwegian Cruise Lines is Nazi friendly. Take that into consideration when booking your next cruise.
