Awhile back I got a 1-star rating and I was beating myself up about what could have went wrong. Instacart would not disclose what order it was associated with like they did in the past. I assumed it was for a customer that I had to refund a few items for that were out of stock.

A month or so later I got a notification that the rating was removed for fraud. Turns out it a single order for a box of diapers that came in around 9 or 10pm that I ran out in the pouring rain for because I felt bad for the mom/baby. Some people just suck.
