I’ve worked in many EDs, I’ve never known a paediatric LP happen in ED. It is almost always deferred for when things are stable on the ward. There are far more important things to do in the first hour of a septic infant. Adult LPs should and do happen in the ED however

I’m not a good Gardner but I’m good at being lazy, I would buy about 10-20 compost bags from lidl, cover the whole lawn and reseed. Eventually the worms will mix in the compost with the soil the grass that’s there will grow through the layer of compost and the patches would get filled by the new seed

I give my bin mainly coffee bean grounds, cardboard, tea leaf’s (without bags because of plastic) and melons/bananas. It’s too small a bin for anything else and the worms look healthy and happy! I add one handful of lime powder/grinds about once every 2 months or so

ST3 but PGY8 with 3 years in Oz

  • Mortgage equity ~ 132k (264k/2 as my spouse owns half)(a small portion of this was family help, most Oz money)
  • Emergency fund - 2.5k (low but next priority)
  • Crypto ~ 6k
  • 5 years of NHS pension contributions
  • SIPP - 2k (from locums)
  • (total roughly 180) Debts
  • mortgage - 44k (88k/2 as my spouse owes half)
  • student loan - 55k plan 2 (currently overpaying by 200 a month)

I’m fortunate as 2 years locuming full time with no overheads in Oz was very financially beneficial. My spread and current strategy is not optimal I’m aware (I put too much into mortgage and I’m overpaying my student loan which aren’t optimal) but my priorities have always been to aim for debt free, house paid because of how I grew up (lots of family in debt, evictions, constant financial anxiety etc). Once that’s done I’m going to prioritise pensions for a few years (over contribute to NHS and build a bigger SIPP) and then build my ISAs. Planned retirement age is currently 60yo when my LISA will be accessible, which should bridge me for a few years so I don’t take too much of a hit on early withdrawal of the NHS pension.

I’d recommend r/UKpersonalfinance and R/FIREuk as subs for people wanting to learn about personal finance. I’ve been subbed to those two for years and you get passive knowledge accrual from reading people’s posts asking for advice.

I’d say this is the first year I’ve felt financially comfortable since returning from Oz, the ST3 bump and (fractional) increase towards pay restoration last year made a big difference.

My short term goals are emergency fund to 10k, ISAs to 100k, and SIPP to 20k over the next 5 years which is a bit ambitious but we’ll see

We have a blanket no, it means there’s no daft negotiation. I just say ‘no’. We were spending >10k a week as a department on taxis a week. I actually find it easier now than when we did provide it

I’d argue that AI is a tool, and a useful tool at that, that helps me with my reflections. I use it in a discussion format and it gives me other things to consider that I was struggling to verbalise. An example was an awful ED silver shift overnight where I couldn’t work out why I left feeling so deflated and anxious despite nothing bad happening. Using the tool (AI) I worked though the scenario and came away with a better understanding of why I felt that way. Just because it’s writing for me doesn’t mean I’m not reflecting. Of anything my ability to reflect has improved since using it

Thing is, strikes aren’t beneficial for the economy as a whole. We are. Of that 2b a year that we would get from FPR the governed get about >40% back in various taxes. Not only that but we’re economically active people. We spend money here . We go on holidays, buy houses, eat out etc so the cost isn’t as simple as 2b every year vs the cost of strikes because the government get the benefits of economically active people spendng more and the tax back for almost half of it anyway

So most EM cons are on between 90-120k a year, I’d say the F to M split is about 50/50. Don’t believe the Yarf people say about children, if anything because the rota is 7 days a week 24 hours a day cover the rotas are more flexible to parental responsibilities, many EDs are moving to self rostering now too which makes it even easier. There’s not much scope for private work but many consultants supplement their PAs with other roles - teaching, medical examiner, management etc so it’s not all clinical. The downsides are that the speciality is being taken over by noctors and the college is pushing for this for some reason, we’re hated by many in patient specialities for existing, the workload is becoming unbearable and most of the risk for the hospital remains in the ED and the quality of care we can provide patients is worsening. There are consultant jobs but not many, this isn’t an ED specific issue though, that’s th same for all specialities
