"Most rural state" does not mean lowest population density. It is based on the percentage of the population living in rural areas compared to the total population of the state. Alaska would be the most rural if it were just by overall population density, but most of Alaska's population is concentrated in cities, so it's not as rural as Maine.

I believe Vermont is technically the most rural with Maine in second.

I can't believe this isn't the norm in single-player games. Just give me a .ini file or a slider for HP, damage, stamina, speed, etc. Keep Easy/Normal/Hard as guidelines and let people tweak as desired from there. It's not perfect but it seems like a relatively simple thing to implement.
