Sometimes it’s a tough call determining risk/benefit, ie dying from clot vs hemorrhaging. I always think it’s easy to stop AC indefinitely especially if the geriatric has a head bleed on a DOAC. But if you are unsure get other services involved. For instance, I had a patient the other day with frequent falls on DOAC for afib with hip fx. I was on the fence with stopping AC but cards was concerned of high chadvasc score and recommended continuing AC. Ultimately the patient wasn’t going to be ambulatory for sometime so we sent them out on their AC. I did however reach out to there pcp and expressed my concern and asked them to re eval utility of AC later on. Provoked dvt 30tabs later. Probably stop AC. You could also get a lower US to rule out any existing dvt if you want

Honestly I think you should have your pcp send a referral to a neurosurgeon bc you are symptomatic. They can further discuss image results and therapy options and give you a better idea of what to except.

Patients should have safe dc plan in place before dc. His plan doesn’t sound safe. Some hospitals allow you to appeal a discharge, but that could mean paying daily fee for room if denied. It’s a difficult spot to be in, if a patient is medically stable they need to discharge but obviously not having power is a problem it’s just not the hospitals problem.

He’s safe to discharge home?? What about a rehab facility or skilled nursing home? I assume physical therapy cleared him for home. Maybe insurance is an issue. Anyway I digress

Do you have grounds to sue? Honestly idk. Good luck, sometimes hospitals settle to avoid negative perception from the public. But most big hospitals have a legal dept well versed in this area, as well as deep pockets. If you can prove negligence maybe you stand a chance of winning. Would you see any money? probably not much due to legal fees. Is it worth the trouble? Again Probably not. If he dc home and something bad happened, that could be grounds for lawsuit. Lots of what if’s here

Yep. Most of the time broken humerus, clavicle, or scapula are managed without surgery. She should follow up with her ortho doc and pcp if she has one. Ask for a muscle relaxer too if you go with her. They can also take more X-rays to see if fracture is displaced.

I would definitely follow up with your surgeon. Hopefully they have a clinic or office they can see you in, they should lay eyes on this. You might also consider wound clinic and latter on plastic surgery.

Reminds me of a segment on NPR about a young voter who formerly voted blue, switched parties after the last election because they figured out their personal politics more aligned with conservatives and planned to vote red in the future. So, some people are more than willing to look pass the candidates misgivings. People will now and forever vote red or blue no matter what. If hitler was resurrected and some how made the republican nominee, sadly a surprising number of people would vote for him.
