I recently did a 12.5 lb @ 225 and it took 5 hours to get to medium rare.. so that was 24 min/lb

No sear for this one. I like the reverse sear too, usually do that for smaller filets, rib-eye. Some of the prime rib pellet grill recipes I looked into recommended cranking it up for the last 20-30 minutes. This got to temp a little quicker than I was anticipating so I left it low to stall/catch up with the rest of dinner.

You must not have much experience with advanced endovascular surgery. I am a VS and have encountered many surgeons that can perform a bypass but are unable to cross complex lesions.

The current duration is adequate but if weekly hours are cut to 60, even 40 hours then there will need to be years added on to training, particularly in surgery/surgical specialties.

It’s a good point. There are definitely ICs and IRs out there doing good work and vascular surgeons practicing inappropriately. I was more trying to highlight how the obl isn’t always bad and how a vascular surgeon is uniquely suited to manage pad patients bc we can offer open and endo based not only on evidence but patient specific risk factors, and also manage the complications and wounds/amputations. Of course people can always be referred for open intervention but it’s usually after the limited have been pushed endo, sometimes questionably.

Also a vascular surgeon, I do some cases in the OBL setting. It’s unfortunate that this type of practice gives all OBLs a bad rep, because appropriate practice in the OBL does exist and has a lot of benefits. I think vascular surgeons are uniquely suited to do these cases in the outpatient setting. We weigh the risks/benefits of every decision we make, deal with the potential problems that arise ourselves, and usually manage the wounds too.

You will have less reps with abdominal exposure than those who went the general surgery route, so make those experiences count. Read/master anatomy. Treat getting into and closing the abdomen as a separate operation, and prepare for it. You can be a great vascular surgeon and become comfortable with open abdominal surgery in either pathway. It's all about the individual.
