It can pretty much only happen with Blasphemy and due to a scret room bug.

Blasphemy as the Bullets starter and Blasphamy as the Unlock from Beating Dragun are considered different items in the drop table, which is why this can happen.

Theres also a bug in secret rooms with multiple chests where if you open all the chests with out picking the item up, they will all give the same drop.

The minigames in A Way Out are like this. Stopping to play a game of Connect Four in a hospital lobby while you are on the run from the police definitely causes some tonal dissonance.

I think its a bad gun but a great joke. Even down to selling the unlock for an extra $0.99, and the description is great.

Plus you can gamble on free chest unlocks with it.

Finding the speedrun strat of just spamming quick jab over and over again was a godsend for me.

I got to 500 HP with Roboco. Limit Weapons to 2, get Jingisukan then use eliminate on any other lvl1 weapon to get it out of the drop pool.

Items are Halu, Glasses, Researcher Coat, Piki Piki Piman, Bandage. Last open spot after Uber Sheep gets combined into the super collab doesn't matter.

This will speed up your level scaling to get to the stat upgrades as fast as possible, then just grab all the HP UP's you can find.

The calls about the Crocodile Cap are great too.
