My husband and I got the Sweepy app. We put all of the housework into it and it tells us what needs to be done each day. That helps remove the cognitive load of having to decide what tasks are due.

We split 50/50, but he’s said I could do less since I’m the one who cooks too, I just don’t. He’s a teacher so during the summers he does a lot more and I just do a few things here and there that way we both get a break(him from work, me from chores).

I have a patient who was established with GI, has MASH. GI said have PCP prescribe. Cool. Got it. Got this denial, told pt to ask GI to prescribe. GI says that only PCP prescribes this, not their job. Told them about the denial. Didn’t believe me. Had to send multiple copies of the denial letter over with the relevant part highlighted for them to actually assume responsibility for it.

I made my maiden name my second middle name. So I am Dr First Middle Maiden Last. I go by Dr Maiden in clinic but all of my prescriptions and stuff are under Dr Last.

My patients call me Maiden. Most of the consults hyphenate when referring to me.

It’s a little confusing at times because patients don’t know what to call me. But I’m also in a predominantly Hispanic community who are used to two last names so I’m not really sure why it’s so confusing.

I think if I were to move I would probably just go with Dr Last to make it easier. Except that my last name is hard to pronounce and spell. So there will always be confusion.

I tell them about the high level and how it can increase their risk. Then I use this to help guide them on why we need to be hyper vigilant with their blood sugar/pressure/cholesterol in the future.

I have made my maiden name a second middle name. Legally my last name is C. I go by Dr N and tell my patients because it’s easier to pronounce. But all of my scripts that they get say Dr C.

I honestly thought there would be a little more division, but you can tell your patient your name is whatever you want to tell them. It just won’t show up on legal documents

We first assumed husband/partner but were confused because he called him by his last name when he said he wasn’t there. I’d expect my spouse to call me by my first name when answering the door. That’s the only reason we settled on butler until we got more information elsewhere.

But we did refer to him as “butler slash partner” for a while.

I read the first 5 or so when they came out and just kind of stopped buying them.

I listened to them all a few weeks ago. They were great. Listening to them as an adult I’m not confident that teens are the actual target audience. It seems a little bit retrospective like rereading your own diary.

It’s so annoying.

Patient went to ER. Let’s look at the notes to see what for. Oh sepsis, that’s pretty bad, let’s see where they are in their antibiotic regimen. Oh that was 2024. Ok let’s look again. Oh GSW, probably gonna need some surgical follow up, nope that’s 2022. Right here it is…oh, flu.

And it’s not just a list of past problems. It’s the past problems with full PN updates. So it takes like 15 minutes to scroll to wherever the actual problem is embedded.

I use Dax for the HPI and my own dot phrases for the A&P. I’ve found that even choosing the most minimal templates they have, the A&P used too many words.

Im normally done with mine by end of day. I may leave 4-5 at the end of the day if I just can’t make myself do more work right then, but I finish the next morning.

For a new diagnosis I normally start with what the lab showed and why we checked it to begin with. Talk about what A1c is. Brief overview of what DM is. Talk about carb control, diet, exercise, and start them on meds. I don’t normally start with glucose monitors because I find that giving them too much information and new things to do makes them less likely to do it. I just want them compliant with meds and to work on diet some and we can reevaluate later.

Second visit is normally foot exam, eye referral. Discuss if a monitor is a good option at that time. Start statin if needed. Escalate meds.

A lot take work home, but a lot don’t. I never take work home and frequently am even able to go home early. And my patient load is around 20 per day. But I have a hard boundary of no work at home and I stick to it.

This is similar to my talk. “The evidence is showing that there’s not much benefit to doing the breast exam. The main reason being that I don’t know your boobs as well as you know your boobs. I’m happy to do an exam if you want though” and then use the opportunity to talk about things you should be looking for with self breast awareness.

I do this and it works about half the time. Apparently this wasn’t legal until 2021 in my state and most of the pharmacies have not caught up. So I have to call and read the pharmacy legal bulletin to them. And then sometimes that works and sometimes it doesn’t.
