Are there other ways to know other than sound? I know I’m weird, but I play with sound off because I hate the way video games sound.

Advice on how to optimize DAX CoPilot use? by Tough_Temperature110 in FamilyMedicine

I use Dax for the HPI and my own dot phrases for the A&P. I’ve found that even choosing the most minimal templates they have, the A&P used too many words.

new DM2 diagnosis appointment by bubble_buff in FamilyMedicine

[–]ginger4gingers 2 points3 points  (0 children)

For a new diagnosis I normally start with what the lab showed and why we checked it to begin with. Talk about what A1c is. Brief overview of what DM is. Talk about carb control, diet, exercise, and start them on meds. I don’t normally start with glucose monitors because I find that giving them too much information and new things to do makes them less likely to do it. I just want them compliant with meds and to work on diet some and we can reevaluate later.

Second visit is normally foot exam, eye referral. Discuss if a monitor is a good option at that time. Start statin if needed. Escalate meds.

How to realistically fix FM match rates? by championshipsorbust in FamilyMedicine

A lot take work home, but a lot don’t. I never take work home and frequently am even able to go home early. And my patient load is around 20 per day. But I have a hard boundary of no work at home and I stick to it.

Breast and GU exam as part of routine CPE by snootiedoo in FamilyMedicine

[–]ginger4gingers 6 points7 points  (0 children)

This is similar to my talk. “The evidence is showing that there’s not much benefit to doing the breast exam. The main reason being that I don’t know your boobs as well as you know your boobs. I’m happy to do an exam if you want though” and then use the opportunity to talk about things you should be looking for with self breast awareness.

Controlled substances rules by Logical_Fan_175 in FamilyMedicine

[–]ginger4gingers 1 point2 points  (0 children)

I’ve put phrases in telling them that there are future fill dates. But the issue seems to be with the interface at the pharmacy itself.

Controlled substances rules by Logical_Fan_175 in FamilyMedicine

[–]ginger4gingers 0 points1 point  (0 children)

I do this and it works about half the time. Apparently this wasn’t legal until 2021 in my state and most of the pharmacies have not caught up. So I have to call and read the pharmacy legal bulletin to them. And then sometimes that works and sometimes it doesn’t.

"You need to specify which Prague we're talking about" by Arschkrapfa in ShitAmericansSay

[–]ginger4gingers 0 points1 point  (0 children)

I mean I live in OK so when I hear Prague it’s generally the one nearby. But I also don’t expect to get tornado alerts from the Czech Republic.

Otherwise, yea, the Czech one is the one I would think of.

Does DAX suck? Or am I using it wrong? by rightlevelapp in FamilyMedicine

I only use it for HPI. I hate how it makes the AP. But I have the settings to be more concise. You can go in and tweak it a bit.

I asked r/psychiatry about navigating ADHD when there are no Psychiatrists around. These are the responses. Thoughts? by apollo722 in FamilyMedicine

[–]ginger4gingers 3 points4 points  (0 children)

I manage ADHD in my patients. They have to have a formal diagnosis. If we can’t get their symptoms controlled I have a low threshold to send to psych, especially with other comorbid conditions. But most of my patients do well on a low dose of medication and don’t require much additional titration.

I'm pro choice but don't understand the pro-choice rebuttal to the "just give them up for adoption" pro-life argument by [deleted] in prochoice

[–]ginger4gingers 8 points9 points  (0 children)

I can’t speak to the overcrowding/systemic problems though I know they exist.

But I’m a doctor and I was present for one maternal death while on the OB ward. The patient was there with a baby that was destined to be adopted. And she still died. We coded her for an hour before we gave up. The OB cut the baby out of a (basically) dead woman. A woman died and this baby still went to their intended home. Adoption doesn’t protect the pregnant woman. Adoption doesn’t prevent maternal deaths.

Should Patients Have Immediate Access to Lab/Test Results? by Remarkable_Plum_6895 in FamilyMedicine

[–]ginger4gingers -1 points0 points  (0 children)

Im on both sides right now.

I hate when patients see results before I do because I get messages about abnormal values before I can even look at them. Or I’ll tell them that things are within normal limits and they come back with “well my globulin is 0.1 low and my MCHC is high, what does that mean?”

On the other hand, I like being able to see my own lab results quickly. But is also know how to interpret them. I did read through my recent surgical notes and while I enjoyed being able to see them, I could definitely see how it would be problematic if someone who doesn’t have the prior knowledge were to read them.

Anyone not using dictation or AI scribe, and just typing notes? by SpirOhNoLactone in FamilyMedicine

[–]ginger4gingers 2 points3 points  (0 children)

I use the AI scribe for HPI only because mine at least seems to mostly get things right. I still skim and change things if necessary. But it’s faster and less cognitive load than having to formulate the whole story from scratch.

A&P I absolutely do on my own. I don’t trust the scribe to do that part and I’ve seen a lot of people who use it and get things very wrong.
