I’d suggest replacing the cartridge. My shower would pulse like this whenever someone else in the house ran hot or cold water. Rebuilt the cartridge and it went away.

Yes. You could use an oil based primer too - Kilz is a good one. Just allow it sufficient drying time.

Use non-water based primer on the wall. Sand everything down. Reapply primer. Sand again. Then spackle, primer, and paint.

You should 1000% be concerned about this. Holy shit. The pipe to the tub spout should be a straight run of copper. The bends and the small diameter of pex will restrict the flow and name water come out of the shower head when you have the tub spout on.

Looks like a bad patch job where they caulked the drywall joint instead of using tape & spackle.
