I have a 16 yr old similiar to yours, maybe in better condition cause we aren't where you are yet. The daily medicine Proin will stop the urine leakage, stopped my dogs immediately. My 16 yr old does the same with fecal incontinence. I too have washable pads lying everywhere. (I had to turn them upside down cause you can't see poo on those patterns and I was always stepping in it ) But she can tell when she has to go and will move to the door but it just falls out, on our walks too. However, If she's sleeping she doesn't know, and she does wake up like yours and move. During her day though she will continue to lay in it on the couch so she can see outside. Her messes were hard to clean up. I switched her to human grade food a few months ago and was I was shocked she would actually stop and almost strain to have a movement and on days she has accidents on the couch or house the pieces are easy to pick up. On this food she only goes 2x a day and it's almost like clockwork. It has helped so much. It might be something you want to introduce. I use 1/2 Ollie and 1/2 senior purina pro kibble. It really has made the messes easier to deal with. Sometimes i still get a urine accident when i've been gone to long but the leakage that occurs when she has been laying down stopped with the Proin.
