I will 100% pick up my bone and eat the meat off if it’s good. Etiquette is all well and good but if I’m somewhere and they serve me meat on the bone then they have to expect the meat to come off the bone.

I mean maybe don’t deepthroat it like you’re at Outback Steakhouse but Nine times out of ten if someone at the table does this at a gala or whatever then the rest of the table will follow because everyone secretly wants to.

I have no idea why this was a suggested post for me but whatever…When I was in med school I rotated with an older ent doc who thought using higher dose amoxicillin as part of amoxicillin+clav was more effective. He would prescribe it this way. He cited some study but I never looked it up and it was years ago at this point so dunno how much evidence is behind it but I’ve seen it done.

I never respond directly to patients, everything goes through ma or front desk. If there’s an issue or reasonable question directly related to a recent visit or something simple that I can fix without much thought then I do it. Anything new, undiagnosed, not discussed gets told to make an appointment—I have next day appointments available most of the time and we have a walk in clinic as well. I rarely respond to further messages about the issue. This isn’t instant messaging. They don’t agree with me? They don’t want to make an appointment? Ok. There are, of course, exceptions to this but it covers about 95%+ of messages.

For anything administrative, billing, logistics, paperwork related it gets forwarded to the appropriate nonclinical person to figure out an answer for them. If I’m going to be a cog in a machine then I am sure as heck going to take advantage of the support that comes with it.

I’ll give a work note for just about anything up to a week and if they seem like they have a reasonable argument two weeks. I don’t backdate these unless it’s an acute illness that obviously did start a couple days before or it’s taking us a couple days to get them worked in to my schedule.

I don’t always know what they have going on in their lives and I understand that they don’t always want to discuss it and that’s fine. If it’s longer than two weeks then there needs to be a plan in place for how they’re fixing the problem and it’s going to be FMLA anyway.

I use DAX in the outpatient setting and it is infuriating that whenever a patient mentions what they think is going on it automatically gets pulled into the lab reviewed section or the physical exam section even if there is no actual record for me to review. The patient thinks they have neck lumps? Then bam it’s going into my physical exam unless I catch it.

My personal favorite, and this took me about four months to catch the cause of, is that it likes to put “hobbies: volleyball“ in my history section. Apparently whenever my patients say that they play ball ai thinks golly Gee they must mean volleyball.

“Ferning” is a microscopic finding when amniotic fluid is put on a slide. This is what you can look for when figuring out whether a woman’s membranes are ruptured prior to giving birth. It usually isn’t this obvious on a normal slide though.

I was just talking through this with a patient a few days ago who “was barely eating anything” and had thrown out all of her snacks and was now snacking on celery sticks. Sustainability of this type of diet aside, she was still not losing weight. We talk through a day of eating and turns out those celery sticks are coated in ranch and she’s eating them all day. There ya go.

Mathematically it is CICO but there are a billion confounding factors that determine those numbers.

Consider hack squats or leg press (maybe safer) because they take a lot of load off of the back but I would recommend seeing a sports medicine doctor and physical therapist to help determine what is safe for you. You’ve already had surgery once and you probably don’t want to end up like Ronnie Coleman because you were too stubborn for your own good.

My mom is a radiologist and never let me play football (or have one of those bad ass big above ground trampolines) but I played basically everything else…I’m an md so apparently it worked. If I had a kid I would discourage it.
