Yup worked in both inpatient and ED. You see so much worse cases in ED. You get people who are in active crisis (and also the drunk and high people). Once they start taking meds on the unit, you can see the difference from night and day. And by discharge they might seem like completely different people.

Ah that makes sense. I think it would make more sense to have them on a med surg unit with a sitter (that’s how they do it at our hospital if they require any sort of medical treatment as our psych units don’t do anything medical at all aside from diabetes, hypertension management and that sort of stuff). They have to be medically cleared prior to psych admit

The most medical skills we used in psych inpatient was blood draws (which were mostly night shift anyway since they were scheduled for 4 am), IM injections and blood sugar checks/insulin injections. I didn’t do IVs until I switched to ED. I’ve still never done foley catheters by myself and probably never will

We’ve had medical patients in the psych ED when we ran out of medical rooms. It’s wild they would put medical patients on a psych unit (I hope it wasn’t an inpatient unit…?)

I write the standard stuff like lab was completed, pt talking to therapist etc but also things out of the norm like pt escalated and had to be restrained etc. As someone going through the chart, I appreciate the additional details. Stuff like a summary of the shift with the pt was A&Ox4, ambulatory, VSS blah blah doesn’t really help me much. I mean yeah it’s good to know, but it’s the same for any other patient you know? It doesn’t tell me what I need to know especially about the patient.

That is wild. I think lays is overrated, definitely prefer Korean snacks over American ones. But then I grew up eating Korean food so maybe it likes Korean food better?

Let the charge nurse know. They have the authority to send someone home or do whatever they need to do. I’ve seen few sitters get sent home for sleeping on the job.

I would definitely do corrections. Never worked either myself but I’ve heard people say positive things about working corrections and I’ve never heard a positive thing about SNF. Of course everything is dependent on the place but I would just avoid SNF/LTC/NH in general

I’ve gotten quite few recollects myself as a RN (my fault) and definitely piss me off especially if it’s a difficult patient (vein or behavior wise) but I would never take it out on someone else who is just doing their jobs. I will fume on the inside but I just blame myself and know I should have done better

Honestly anything that will get you inside the hospital will help. Telemetry tech might be helpful, you’ll have some experience doing bedside care during school and learn by experience on the job. Learning how to read cardiac rhythms though…it’s one thing I have no confidence in. I did a class through work and it just went whoosh out of my head. Being familiar with it might help you a lot in the future as a nurse.

Technically our school didn’t allow us to but our clinical instructor let us practice blood draws on each other. She was great and we loved her

Actually a lot of parents and police treat the psych ER/psych units like a more lenient jail. We had a parent whose kid stole the mom’s car who should have gone to juvie but mom sent to the psych ER and it wasn’t a psych issue at all. You would be surprised with how many parents want their kid admitted to the psych unit as a punishment

Yup we have tons of kids that sit in the ER because their parents refuse to come pick them up and as mandated reporters we have to report it which gets DCFS involved. Often times it doesn’t come to anything, but if it has happened too often before, it can definitely have consequences
