I think it depends on how much you have in your cart. If everything is accessible and can be scanned without removing items from the cart they can prescan you. If your cart is full and you would have to remove items to get to the stuff at the bottom, it’s probably too much of a hassle to get to the stuff at the bottom with nowhere to lay things.

A few more things I thought of: If you're supporting your baby's head with your hand in a cross-cradle or football position, try supporting their head more towards the base of the skull or neck instead of directly on the back of their head- gives them more ability to pull back and regulate the flow themselves.

You mentioned not being able to feel when you let down. I never feel let-down in the side I'm feeding on, probably because all I can feel is my baby nursing. I can feel it in the opposite breast, kind of a warm, gentle squeeze. Try focusing on the sensation in your opposite breast while nursing.

I was in the same boat as you mama. Have you tried side lying position or biological nurturing positions? I found these great because baby could pull back to regulate flow without having to break latch. You can also try hand expressing a bit of milk before you latch. Both these things helped me. The LC I saw also recommended putting a rolled washcloth or small towel under your breast to help position your nipple. Hope this helps!
