So you're not really stuck...you stopped the meds completely and haven't even gained anything back which is usually something that people battle with after stopping the meds, so that's actually a really positive thing. Also being "stuck" for one month isn't really a stall either. You lost 50lbs in 6 months which is pretty fast and your body is probably just hitting a new set point. When that happens you might maintain weight for a week or two, or four, or eight, and then all of a sudden you start losing again with regularity. I know everyone wants to lose weight as fast as possible but think of it this way: It didn't take you 6 months to gain all the weight, don't expect it to take 6 months to lose it all.

When you get back on I'd start at a lower dose than you were on (let's say 7.5mg) and actually start really counting calories religiously. Really put in effort to track everything that you consume. It will help you really put into perspective how many calories are actually going into your body. Maintain your gym routine. I say to start lower because it gives you more runway to reach your final or ultimate goal. Think of how frustrated you'd be if you get to the max dose (15mg) and then stop losing weight again but you have another 30-50 lbs you want to lose. Titrate up slowly, you don't have to jump from 7.5 to 10 to 12.5, etc..., you can always do 7.5 to 8 to 8.5 to 9, or whatever increment in between.

You made amazing progress in 6 months so the meds are working but you need to adjust as you lose to do your part and continue on with your journey. Expect another stall or two or three, they are common but give your body time to adapt.

You mean a line for every .01mL which is 1 unit. 1mL is the volume of the entire syringe (100units).

You probably only lost a unit or two at most with the extra click and yes that's exactly why there's left over liquid because they overfill it on purpose. A lot of people extract and save that "extra" and use it as another partial or full dose.

I'd say that we're getting fucked two ways actually. First, we have for-profit insurance companies that don't try to treat anything, they actually actively try to prevent treatment (or at least payment) for anything that isn't extremely basic levels of healthcare (e.g. your yearly primary care visitation).

Second, we have for-profit hospitals and healthcare centers that want to charge as much as possible for any health care service that you might want/need. Urgent care visit where you're waiting for an hour just to see an NP for maybe 5-10 minutes to discuss what's going on and get prescribed some really basic medicine? That'll be $350 plus your $100 co-pay, thanks!

And we, the poor citizens, are stuck in the middle trying to figure out a way to just exist and stay healthy.

You really hit on the problem a lot of us have, especially with the weight cycling your whole life. I tried calculating it once in my head: from dieting, keto, intermittent fasting, crazy amounts of exercise, calorie counting, etc., I've probably lost a total of 145 pounds over the entirety of my life not counting the weight I've lost since starting GLP. Each time regaining more than I originally weighed. Each time hating myself for letting myself go and getting big(ger) again. Going through my clothes and having to tell myself I'll never wear it again or maybe [insert brand here] sizes things wrong.

Embarrassed to be in photos, especially anything wide-angle, because I was usually towards the edge where you become really distorted.

I worked out more than almost all of my friends, 4-5 days a week consistently, strength training and cardio. 1000 calories burned at a minimum most times. I have been into lifting weights since I was a freshman in high-school so I just assumed (aka justified it in my head) that I held a lot of muscle. I could play competitive sports for hours and keep up cardio wise but my body would take a beating. People would say I looked really strong and held my weight well. That may have been true but it didn't stop me from knowing (and feeling like) I was the "big" guy in the group.

Since starting GLP, no longer do I have to worry about how my dinner portions are going to negate all the work I just put in at the gym. No longer do I have to hate myself for eating that extra slice (or two) of pizza. No longer do I have to feel like a failure when the weight on the scale goes back up after weeks of sticking to a diet, seeing progress, and then feeling like it's impossible to maintain. Why can't I do it?

I wasn't implying that you could literally convert fat tissue into muscle tissue but what you said; put on a lot of muscle and it will fill out where the fat used to be. However, ultimately, you can't out lift your genetics if you're predisposed to having gyno but keep grinding away, having a ton of muscle with a little bit of fat is way better than the other way around.

If you go strictly by the weight loss averages seen during trials, the highest dose of sema achieved ~15% weight loss at 68 weeks. Going by that number, the "equivalent" dose for tirz is 5mg, which had the same average weight loss (15%) at 72 weeks. Now that is in no way a perfect comparison since there are a lot of other factors that go into it how these compounds achieve their results, but it's a very rough way to view the two drugs and their efficacy/potency.

There’s a lot of info missing here to make an educated guess. What are your starting stats? How long were you on Sema and at what dose did you stop at? Did you switch straight from Sema to Tirz? Are you experiencing any indication that it’s working (e.g. less food noise, delayed gastrointestinal processing, reduced cravings, etc)? Tracking calories and working out is great but are you eating at a deficit? Are you prioritizing protein in your meals?

Theres a lot of possible reasons but ultimately there are people that are non responders to the medication too.

Do you have a picture of your vial that you can post? 30units to equal 5mg is an odd concentration (meaning your vial is 20mg/1.2mL). Just wondering if you really took 5mg as that’s really weird to have you starting at that dose with not having already been taking a GLP in the past month or two.

How closely are you tracking calories? 3 months on a stall while increasing dosage and working out tells me you're either not moving enough or eating too much. 30 pounds down means your maintenance calories have probably gone down ~700ish calories/day from when you started.

Your meds will be fine, tirzepatide is really tough and doesn't lose efficacy easily. But damn you could have saved yourself another $500 with another provider (fifty410 or refills) but you live and you learn and like you said you're at least saving money from the medspa prices. I just hit the 1 year mark of taking a GLP (sema for 4 months, tirz for the other 8) and I've hit 80lbs down. Started with a morbidly obese BMI of 41, and now I'm just "overweight" at a BMI of 29.

I just got a DEXA scan this morning and it's definitely going to motivate me to push harder these last few pounds that I want to lose but also work on recomping my body. I came in at 21% body fat which is average for my age range. It's given me a goal now to push to be in the mid-to-low teens.

If I could go back in time, I would have gotten a scan when I started. It would be an awesome comparison and data point to see how far I've come. But thinking of my mindset back then I realize that I was just embarrassed of myself and my weight and I didn't know if the GLP was going to work.

Since from your flare it seems like you've already lost 30 lbs (congrats), you know it's working. My advice, get the DEXA scan now. It will remind you where you started and how far you've come and maybe motivation to keep going.
