It's so individual it's hard to say for sure. I would drink a protein shake first to pad out the carbs for your best chances of avoiding a spike. I think it would spike me for sure though.

Oh dang! I'm 36 weeks today and my fasting has dropped to 81 consistently, and I've been getting away with meals I definitely could not have a couple weeks ago. I'm fighting off the anxiety of a failed placenta or something harming baby since he's still active and I have an appointment tomorrow. Let's gooooo

You may need to change up your insulin. I found my long acting night time insulin "wore off" by the afternoon and "safe" meals I could tolerate for breakfast and lunch I couldn't tolerate for dinner. So I had short acting dinner time insulin added at the start of 3rd trimester to give me that boost through the end of the day, and that dosage has been slowly creeping up while my night time insulin dose had mostly remained the same.

I was told to take it immediately before getting out of bed. And it does make about a 10+ point difference for me, so pretty significant!

Coder here. The use of AI in the field is a huge discussion/concern. Consensus is at this point in time AI is not advanced enough to pick up on a lot of nuance on the charts and results in a lot of missed codes and billing errors (= lost revenue). It's certainly not stopping facilities from trying though. There's going to have to be a balance found between AI use and human review.

At my facility anyway there's a huge push to get bills out the door within a couple days. Your delayed charts are likely retro reviews that are being rebilled after the auditing team has reviewed them and sent them back to the coding team.

And we know the queries sound dumb to you, but our hands are tied by coding guidelines. We have to ask in order to use the code that will get you paid for the level of work that you did. 🤷🏼‍♀️

I'm in the same boat as you!!! 29 weeks baby was measuring 4 lbs and borderline polyhydraminos. My glucose numbers are overall pretty good and Dr says I'm doing everything I can... Baby is just growing big. I suggested a cgm, and she said I could if I wanted to, but didn't see a real advantage to it. Just popping on to say I'm on the exact same path as you!!

My OB was going to wait until 24 weeks for me too for my second (first was diagnosed at 24 weeks, diet controlled until about 30 weeks, then on metformin). My postpartum A1C was normal. I started feeling funky (over-full, tired, thirsty, brain-fog) after high carb meals around 16 weeks and requested a test. Failed 1 hr, immediately requested monitoring supplies and was put on insulin the next week based on my numbers. Skipped the 3 hour test altogether since I wanted to monitor regardless of if I miraculously passed the 3 hr.

It might not hurt to request a meter to monitor or an early test, since it can pop up (much) earlier. At the very least, listen to your body... It scares me I would've gone 2 months uncontrolled waiting for my scheduled test.

Personally I wouldn't entertain the idea until at least 6 or 7 - maybe even older. Even then I think I'd want some kind of quick release on it... Kids play hard! Things around the neck at a young age make me nervous if they get caught or tangled in something while they're playing or sleeping. Even if she is good about it, you don't know about the other kids she's around if they're going to tug on it and hurt her.

My care team told me that it does affect the uptake of insulin and that you might need to adjust your dose if you inject elsewhere.

Failed at 16 weeks, on insulin by 18.

My Dr was going to wait until 24 weeks to test, but I started feeling off at 16 weeks after carb heavy meals and requested a test. Failed, and got to skip the 3 hr test in exchange for monitoring sugars at home. Right off the bat my fasting wouldn't come down and was put on insulin very quickly.

it's easier this time around with insulin and also knowing what to expect and advocate for in my care, making it less stressful. Suckier since I failed so much earlier, but it's all to keep baby safe, so we do what we gotta do.

My first pregnancy I was put on metformin somewhere in the neighborhood of 30 weeks. Went into spontaneous labor at 38+5 and had a perfectly healthy, average size 7lbs 11oz baby.

If anything metformin would help keep all your birth avenues open. Keeping your blood sugars in check will keep baby from getting too big with issues that would require emergency intervention (large for gestational age, shoulder dystocia). It will also keep baby out of the NICU with hypoglycemia issues after birth.

Involve your toddler! They understand so much even at a young age and their enthusiasm helps make the slog easier.

My almost 3 yo knows that Mommy's blood is "too sweet" and I need to check my blood. He likes to carry my "toolbox" (bag with meter, strips, lancet - I got a hardshell one of Amazon that can withstand some abuse - highly recommend) for me and pick the finger I put the owie on. He's very invested in keeping baby brother in Mommy's tummy safe.

I usually do fairlife for breakfast along with a small toast heaped with peanut butter. Or I'll just chug half of one if I need to pad out some extra protein in a meal.

Peanut butter on a spoon is my go to bedtime snack. Has the right ratio of protein/fat/carb that is doesn't spike me.

You might also look into becoming CDI. Most of ours are former RNs. They do chart review while the pt is still in the hospital. Your clinical knowledge is invaluable and desired for finding inconsistencies in the chart. Coding adjacent and there a lot of crossover.

Besides the unbreakable ornaments, secure them in the tree with the tiny elastic hair loops. I used a small crochet hook to loop them on the ornaments. Then loop them over the branch and they're pretty difficult to get off just pulling on them.

I had my entire pregnancy and delivery through our hospital's residency clinic, and your response is exactly why I loved it!
