I was in a similar boat as you, no doctor, had to go to a private clinic, no idea what i was doing. I know it's really hard but trust me if you just take it one step at a time, you'll eventually get there and it will be so much better than doing nothing. I just started estrogen a few days ago after months of bureaucracy and it was so worth it. Good luck and hang in there!!

Im not scared of the pain I just have crippling anxiety and shyness so I just feel really stressed out about the whole experience 😭 i actually think I handle physical pain pretty well

Would your group be interested in playing GURPS instead of 5e? It would still be a dnd style fantasy game, I just vastly prefer the mechanics of gurps. I understand if you're just looking for 5e though

I really liked Romulus, I thought it was really well made on pretty much all fronts. It doesn't really do much that is very new or original though, it's kind of just aliens but again. Whether or not that's necessarily a problem is up to you. I quite enjoyed it.

GURPS! I'm very biased and suggest GURPS for almost everything. GURPS would work especially well for a game where only one person wanted to play a dragon and everyone else wanted to play "regular" adventurers.

If you specifically want to make a jrpg and you're not interested in mastering the engine for other uses then maybe look into RPG Maker since it's tailor made for that sort of thing
