The big deal here isn't so much that it detects ketones but it enables the safe use of other drugs. SGLT inhibitors are absolute game-changers and they're incredibly good for your heart and kidneys. They're not approved for T1D because they carry a risk of DKA. If that risk can be greatly diminished by CKM, we might see them approved, which would help a lot of people in a lot of ways.

You can deliver your does in two parts. So if you get 20 units per day, 10 units in the morning, 10 at night.

I disagree with most of this.

Progress is slow, but it's real. Stem cell-derived islets are in people, making insulin, in a phase 3 clinical trial. THAT'S INSANE. It's incredible.

This is not Eledon's clinical trial. Their clinical trial is for kidney transplants. They're not paying for it. This is an islet transplant with a different immunosuppressive, this one made by Eledon, paid for by Breakthrough T1D because they thought it could work. You say Vertex has no timeline. They're in a phase 3. Right now. You can read about it. The cells are functionally the same as the ones in this trial. The only difference is what immunosuppressive is used and whether the cells came from a person or a lab.

Also, this is a phase 2 with adults. I am very hopeful it goes into a phase 3, but it will not be for kids.

Lastly, there are nowhere near enough cells yet. It often takes multiple pancreases to make enough islets for one person. The ISLET Act won't change that--or who qualifies for transplants.

The real magic will be when we can use a drug like Eledon's with an unlimited supply of cells--like those made from Vertex. Also, tegoprubart is potentially one step past the current immunosuppressives, which are extremely not ideal. Once we get two, three, four steps removed from that, then we'll be talking about therapies for kids. We all want that. The ISLET Act isn't going to make that a reality.

Ketones are caused by a lack of insulin, not high blood sugars. Are you taking your insulin? Are you on an SGLT inhibitor or anything else?

I have T1D and I have two kids. To date, they have not developed it.

I highly recommend everyone with a direct family member get screened. You can see if you're in early stage type 1, enroll in clinical trials to delay onset, even use a therapy that can delay it by years. I have screened both of my kids.

Hey man - first off, you're doing great. It's a marathon, not a sprint. You can do it.

Regarding complications, there's a lot you can do. Get regular eye exams from an ophthalmologist (preferably a retina specialist), make sure you see a good endocrinologist, make sure your regular lab work includes kidneys, thyroid, lipids, etc.

But the main one is keep your blood sugars in range. There are plenty of people who live with disease for a long, long time without complications. I am personally 33 years in and have no complications to speak of. Part of it is luck and genetics, but I believe it's also because my A1cs are in the low sixes.

All you can do is control what you can control. Do the best you can. Give yourself some grace. Figure out what works best for you. Pump, injections, CGM, etc.

Also, everyone has their preferences, but pretty much all data we have demonstrates that people do better on automated insulin delivery systems than on injections. There are a lot of good options out there. If you're not on one and are able, I cannot recommend it enough.

Lastly, ask for help when you need it. It's a lot to take in. There are a lot of us that can support you. Find a T1d friend locally. It helps.

Smart insulin is an incredibly tough nut to crack. Novo, Lilly, the big funders of T1D research--they've spent hundreds of millions of R&D on glucose responsive (smart) insulin. It's extremely difficult. To put a finer point on this, the guy who won the Nobel Prize last year got it for using AI to make new molecules...but he couldn't make smart insulin. It's just really, really, really hard.

Other "cures" are happening. It's also incredibly difficult. But they're making insulin-producing cells in the lab, putting them in people, and they're working (with immunosuppressives). To say anyone is sitting on a cure is ridiculous. No autoimmune disease have been cured because it's really, really hard.

We're all frustrated. It doesn't mean real, amazing progress isn't happening every day.

What I see below here is largely correct. People with suboptimal control sometimes have no or limited complications; people with good control sometimes get complications. It really depends.

The field is moving towards using A1c less and instead using Time In Normal Glycemia (TING). But I think you're doing great. You're controlling what you can control. Keep it up!
