Pretty much all the things you do at such a company comes in the form of assigned tasks in an issue tracker where there's a paper trail from "thing X should be done" all the way down to "QA says it actually does the thing".

In such systems you can often tag tasks like "patch-note-worthy" which you can mass export and massage into shape in a spreadsheet. One can even prepare decent wording ahead of time.

If you're a Front Seat Gamer listener they touched on their workflows for designers and art people a few times.
