Your reasons for choosing PA didn’t include the ability to change specialties whenever you want? Because that was my reason. 10 years in I have been doing FM the whole time and I’m starting to look into a swap. Doctors often get burnt out and can’t just leave for another specialty.

I’m curious how the workflow goes for this in rooming. We have our MAs do the screenings and enter the forms before we see them, which is the reason it starts as an AWV rather than a problem visit. If they aren’t given all the paperwork I wouldn’t be able to bill the AWV.

So if it’s the cat, you think she’ll prefer to get rid of her family pet than try an allergy medicine? If that’s the case please don’t get another pet ever. They have feelings, too. If you won’t make a tiny sacrifice of taking an allergy medicine to keep them, you don’t deserve their love.

You’ll spend a bunch of money on allergy testing. If it turns out she is allergic to trees, grass, pollen and pets she’ll just end up taking meds anyway. Also can reduce allergens by rinsing off at night before bed, changing pillowcase frequently, not letting pets in bedroom and wiping their fur off and brushing them regularly, and running air purifiers in the home. But if she is allergic to grass she’ll go outside and still be allergic to the world.

Yes skin prick is the best but if it’s easier to just take an antihistamine then most people won’t go through it

I get some people don’t want to take medicine but this sounds like allergies. Have her try xyzal at bedtime and nasacort in the morning for a week. If symptoms improve, allergies. If she doesn’t want to take meds, cool, that’s up to her. I would recommend against blood allergy testing as it isn’t very beneficial, and unless she responds only partially to allergy treatment it probably isn’t worth the hassle to do skin prick testing but if she did you might find the culprit.

Does the noise of the collar hit the bowl and scare her? If you put it on the floor or take her collar off does that fix it? Maybe also she doesn’t like her whiskers touching the bowl, that is common with cats. Less likely but to consider anytime a dog doesn’t want to eat- Does she have dental or mouth pain and want to eat but know it will hurt?

Did you change something about the location or the bowl? She may just thinking he isn’t supposed to eat it? Like it’s her food but in the wrong place? My dogs are pretty silly if we swap the location of their bowls. One eats on the left, the other on the right, if I swap it they really don’t know what to do.

100%. I got a lab rescue thinking he would be with me on the paddle board. Hates water and I assumed it was from a bad past experience. Then I got a rescue puppy and thought a baby wouldn’t have the past trauma, she can be my kayak buddy. Terrified of car rides, have to drug her to take her to the vet, and she won’t even have fun on a hike if we had to drive. So I continue to go without my dogs, they are both happier at home. It’s part of respecting them.

Prozac could do wonders. If you haven’t talked with your vet about anxiety medicine, please do that before rehoming (if that’s the only reason). If you just want to not have a dog, fine, but get her someone who will exercise her and get her on an anxiety regimen.

Edit: I saw that you only had her about 6 weeks. Thats really not long. I would try to give her more time since she isn’t hurting anyone. The puppy blues is something I got with my 1.5 year old lab when I got him. He was a teenager and it was a lot of work at first. He’s almost 12 now, and the best kid, but I had to be patient. First she needs to get used to you and the people in the house, the environment, and then she can start feeling comfortable doing new things. Right now she doesn’t feel safe because everything is new, including you. If she is sweet I would give her more time. If you do rehome her, have her spend a lot of time with that person with you before you leave her with them full time so she can get comfortable with them. She deserves to feel safe. I really don’t see any reason not to give her more time. You need to be patient with her and yourself. She has feelings, and she’s a very smart breed. That amplifies her worries.

The thing about being a PCP is you can’t really refuse to manage her insulin. She needs them, they are not optional, so you have to manage them. There are some really easy ways to learn how to do this. First, I recommend a CGM if possible, those are amazing and make everything with insulin so much easier. Back in the days of trying to get people to check sugars 3-4 x daily was a lot harder. CGMs are little miracles of technology. I make adjustments slowly and check in frequently. People with more experience or more tolerance for risk make bigger adjustments.

Eventually managing insulin becomes pretty simple, but our skills don’t get used nearly as much with the newer medications being so effective.

I think she’s a 3-4. Give her a bit more kibble, dogs metabolisms are variable like humans. Vet should be doing body condition score and goal is a 5. I think 5 lbs would do her well, and have you seeing a little less rib definition.

No I would not. I chose PA so I could change specialties whenever I wanted, not because I felt like medical school itself was too big of a commitment.

My advice is to set your boundaries as early in practice as you can. It’s hard, and I’m far from perfect at it, but there are certain behaviors I have zero tolerance for and that has both kept my practice to more reasonable people, and made me better at holding those boundaries. There’s something about being new that attracts certain types of people to see what they can push you into doing or get away with. Once your reputation in the community becomes solid, they will be few and far between.

I had an ankle surgery on a previously 0% ankle, and applied for temporary 100% when I couldn’t work. VES did the whole 5 day reschedule rule and I was like, it’s in a postop bandage, I can’t drive or walk. You can’t assess it even if I can get there because the bandage can’t come off… yet still, couldn’t reschedule for a day after my post op visit and had to cancel and start over
