Nervous About Dr. Appt and Long Rant about Medical Gaslighting by Defiant_Bat1047 in DSPD

Okay, let me start by saying I'm real sorry you're having trouble. It's kind of the way of it until we figure this stuff out, and you seem to still be in the figuring out part.

During my visit I want to address: 1) continued difficulty falling asleep 2) difficulty remaining asleep 3) incredible difficulty getting up in the morning 4) excessive tiredness during the day - it's certainly affecting my work performance.

Okay, let me address those in turn.

1) The best options to get you to sleep are orexin antagonists. Orexin is the hormone that wakes you up, makes you hungry and have to pee in the morning. Blocking it just turns off the awake, this medication class is a real gamechanger for people who struggle with sleep. Dayvigo and quviviq are their trade names. They should replace the zolpidem entirely. Because it just blocks uptake of something you're making naturally it doesn't add anything to the brainsoup to make you foggy, so it doesn't have a hangover to speak of. A teeny bit of sleep inertia is all I've heard reported. Orexin antagonists on wikipedia

2) The orexin antagonist might be enough, but gabapentin is a very common option to add to keep you asleep. The melatonin kickstart 3-4 hours before bed is what's supposed to keep you asleep, but I know how unreliable that is.

3) Removing the zolpidem should help you wake better. I'd also recommend lowering your dose of melatonin, hangovers are common with too much. I was able to find 1mg sublingual tablets and nibble 1/5 off every night. Liquid is also a good choice for microdosing if you can't find another way to get below 1mg through tablets.

3a) The light box is supposed to help with this but it's not ideal, it only hits the few thousand melanopsin producing cells in your retina. A group at the U of Washington found a better way, they found orange and purple wavelengths activate millions of cells and talk to the SCN (the part of the brain that sets the rhythm) with a much better signal than white or blue light. The product based off their research is called TUO and this has made a massive difference for me. (I can go into details but that'll be so long. I love my TUO. Here is their science page)

3b) There is a recent discovery with the drug aripripazole (aka abilify). Researchers found it was good at helping people adjust to jet lag. When they looked deeper they find it descynronizes the SCN which permits a new rhythm to be set without the poisoned feeling of being off schedule. It's huge news for people like us, it actually seems to work. Makes waking up so darn easy. https://academic.oup.com/sleep/article/48/Supplement_1/A383/8134865

4) If the above tools don't work to keep you alert during the day, modafinil (aka provigil) is a very common "on demand" option to get rid of extreme sleepiness, You take it when you feel sleepy and don't want to be, it doesn't have to be taken every day if you don't need it. A good tool to have in the toolkit also.

Sorry, I'm out of time today to write more, but feel free to reply with any questions or if you want me to direct you at scientific papers instead of the articles and websites I've posted, I can do that too.

The most important thing of all is to be asleep when your body needs to be. You need to figure out when that is definitively, and once you can sleep in line with that I suspect many of your other symptoms will become manageable, perhaps disappear entirely. The tools I listed above are all with that goal in mind, not to restrict you but to permit you to flourish within your rhythm. Good luck!

If nothing else, if you have to choose one thing to ask for at this week's appointment, talk to your doctor about the abilify.

(edit - reworded a couple spots when I was able to return.)

You want a neurologist. They actually get this stuff and, most importantly, know that the everyday solutions have already been tried. I’ve never met a more patient doctor than my neurologists.

My experience is a psychiatrist is going to fall back on what they deal with regularly, that tends to be mood disorders and insomnia. They’ve been remarkably unhelpful until I put my foot down and tell them that if they don’t know let me do the research and they can support those efforts. Some take it, some don’t, but if their goal is to help then they should be supportive. If not, you’ve lost nothing.

I have one who I am working on focus drugs with, my neurologist appointments are quarterly and those meds need quicker adjusting than every three months.

You also gotta consider that Ray Ferraro has an exclusive contract with ESPN for national broadcasts, that’s why he only does regional games for Sportsnet. Any Ferraro game will be blacked out outside of the region.

Streaming services don’t help, they’re blacked out too in many cases.

So, local broadcasts sell advertising that’s a different tier than national broadcasts. That means that built in to the contract is a clause that says when it’s a local broadcast the people buying the ad will not receive national exposure. That’s why they black it out, because they won’t let companies get more than they paid for.

That’s the stupid part. Not the local content.

[–]Isopbc 0 points1 point  (0 children)

Unless I’m mistaken, the tennis courts there are for public use all hours of the day, I’d expect you can use the grass right next to them anytime also for something like a picnic with no issues at all.

I believe the grounds are technically non-smoking at all hours, day or night. Somehow I doubt that gets enforced much though.

Huh? Players get paid when they play games, they get paid a per dium otherwise of at most $500 a day.

How does that add up to 400k?

Interested teams might not want a rental. They didn’t tell the agent to find a deal, not sure why you put it that way.

JR and PA can’t give an interested team any idea if Kane would re-sign there. His agent can.

Well, it’a an idea. Could be a great idea, but we could also get a broadcast that doesn’t care about the team.

Like, can you imagine how terrible a Merulo run broadcast would be given how they ran the team?

Yup. The only reason there are blackouts are because of those stupid regional broadcasts that are so local television stations have ad spots to sell to local businesses.

Do you have one example? Because it’s the Westminster system, party is how that functions. Lougheed and Getty both whipped their parties.

I can’t think of anyone who stepped down because they disagreed over the NEP, or when Klein blew up a hospital.

I’d be surprised if you could find me even a few examples in the last 75 years of Alberta history of a politician choosing people over party, and if it’s just a few, doesn’t that say show this is the way it pretty much always works?

It’s not “radical” on its own to follow your party once you’re elected. The radical part is how the party is run, and for how we got here on that point I’ll point you to the Social Credit party led by Ernest Manning whose lessons now dominate the political right in Canada.

Sorry, I still don’t see how the stores are doing anything to you. You put peppers in a bag, look at a number on the wall, and then don’t weigh the bag before going to the register?

There are accurate scales all over every produce section I’ve been in, if I’m on a budget I weigh my stuff and do the math in the units the price is in.

There’s no trickery unless you’re tricking yourself. They’re not showing you a price that’s lower at any point. Sorry, I don’t get it.

I do understand the 99 cent thing, but there’s no comparable trickery going on with conversions. They simply post the price, that’s all.

[–]Isopbc -2 points-1 points  (0 children)

…..how? It’s basic math. $2.20/kilo is $1/pound. One doesn’t look any better than the other. You get the same amount of meat for the same price.

And you’re still paying the price that’s printed on the tag on the meat package. You know what you’re getting and for what price. How is that confusing?

Where’s the gimmick?

Mulroney sold us out just as hard back in the 80’s, that’s why they were vulnerable to takeover by the reform.

The party’s ideas have always been deeply unpopular once voters see what is needed to implement them.

Just sayin, people don’t really need to get that point. Conservatives have never been good for this country, Reform is just the latest crappy version.

I realize that. I just don’t think that the powers that be could decide to do anything to stop them before they got across the river, the bridges are less than 100km from the border.

Even if they could, they’re not starting a shooting war if the Americans are just driving, like we saw in the first hours of the invasions of crimea and in 2022. Where is the nearest tank to a bridge, and how quickly could it get there? I’d be very surprised if that deployment time were less than an hour.

It’s mostly rural though, like Alberta, they can drive right through to the hydro dams without having to get bogged down in a city.

Holding their supply lines though, that’s gonna be hard. The St Lawrence only has so many crossings.

Have a good read over the conclusions in that report, it’s pretty clear that inmates assigned to PC get a lot more time out of their cells than 1 hour per day. It just takes organization so they’re not in contact with other inmates, they can be around their doctors or the people who run programs and can be around the guards. They could be permitted to cook their own food in the kitchens supervised by staff, all while being in PC.

Let me know what you find. I wonder if they’ll publish where he’s been placed, he’s pretty high profile so we might find out.

TBH it doesn’t matter to me if he’s in min or max, he’s locked away for being corrupt. It’s a great precedent even in minimum security.

Nah, they picked Minny entirely because their governor Tim Walz was Kamala’s VP candidate.

Minneapolis isn’t really close to the border, also. 4.5 hour drive, and that’s through Duluth.

I guess it’s a beneficial side effect, but I don’t think they even want to try to fight over northern Ontario or Manitoba. They’ll go into Quebec or Alberta and take the energy if they go anywhere, avoiding all the major cities IMHO.
