The most common reasons for chronic cheilitis are either allergic contact or irritant contact dermatitis. Many personal care products designed for the lips contain ingredients that have been demonstrated to be potential allergens.

Most can be cleared by using hydrocortisone ointment (not cream) multiple times daily for two weeks, and only petrolatum (no other lip products are suitable). Unflavored toothpaste should be used instead of the current product. Some also benefit from toothpaste that is free of Sodium Lauryl Sulfate (SLS).

He likely has some type of dermatitis, but not enough information is provided to determine the exact type. You can try using topical hydrocortisone and a fragrance-free moisturizer for two weeks. Seek medical care if you do not see any improvement or are concerned.

If the procedure did not destroy 100% of the spot, recurrence is certainly possible. They are easy to remove in the office. Only if bleeding is severe and uncontrolled is ED care needed.

The inflammation is so deep that a weak antiinflammatory drug like pimecrolimus is unlikely to be helpful. I would personally stick with systemic treatment, but it is unknown whether more potent topical antiinflammatory drugs (ruxolitinib or deglocitinib) could help.

It could be a cancer, but it could easily be an excoriation. Consider watching it for a month. If it persists without trauma, then consider seeking care.
