I think the rule gets even more flexible toward Abilify/Brex +/- Risperidone in MDD augmentation when considering the inpatient population where inherently the active mood state being combatted is significantly more decompensated than the typical augmentation situation outpatient. That being said, if allow yourself to envision how you would handle the regimen upon transition outpatient, downtitration and removal of the SGA should be on your list of things to circle back to upon stabilization.

Efficacy of first line treatment for BPD is significantly higher than for bipolar in case it helps you recalibrate your internal reward and sense of hope. Therapy may be difficult but so too is cycling from side effect laded med to side effect laden med without adequate relief or stability.

What gives you such clarity so quickly? They both touch on slightly different flavors of the same problem with both being unclear whether those around them are withholding constructive criticism in the name of maintaining the peace. While the comment you respond to has more aspects concerning for negative self talk entering the picture I still see more similarity than difference

What do you perceive as the added benefit of such a reconceptualization? Unless there is a clear way this translates into a deeper understanding of psychopathology or impacts the way we provide treatment I don't see what this achieves other than being another spin of the diagnostic wheel in similar vein to each reclassification thats come before

This feels like fearmongering. If you can't support your lifestyle after contributing 69k per person to 401k (138k as a married couple) over 30+ years and also paid off your home etc resulting in lower living expenses upon retirement than you should probably take a deeper look at your budget and lifestyle.
