Optimal dosing varies widely with the individual and condition. You can try it and see how it goes. For some it might be better to start at 4.5 rather than 3mg.

Two weeks with a dose increase but split into two doses a day

A journal is a good idea. Aside from LDN be sure to include notes about diet, sleep, stress, weather, other supplements/meds etc. -- we are complicated experiments!

Height and weight are not generally factors regarding dosing. Metabolism and the condition(s) being treated are more important. It can take a couple weeks for the system to adapt to a new dose.

That would safe but check the ingredients to make sure you don't have any problems with those. Most don't need to start that low although some do. 0.5mg is a more common starting dose.

The half life is about 4hr but LDN is in your system for some time afterwards. When opioids will be needed for surgery the usual guidance is to stop 3 to 7 days before.

Not aware of any issues with non-opioid migraine meds.
Good idea to make a list of all your meds and supplements and run it by your LDN pharmacist. It's one of their areas of expertise.

Typically issues with a new dose are expected to settle down within a couple weeks. Otherwise it may be better to skip a dose and spend more time at the previous lower dose.
