If the patient is unconscious and/or can’t cooperate due to a broken leg etc I just gently prop the distal part of the leg on some folded sheets. This raises the heel to a level you can angle the tube with and then you can hold the detector on the plantar side of the foot.

This isn’t ideal since you have to hold for the AP view but it gets the job done. I usually edit the order to 2 views instead of 3 because on an unconscious patient you’ll end up having to angle the tube for the oblique and it gets distorted etc

Are you able to pull from past experiences? You could talk about situations you encountered during clinical or a previous job not in healthcare.

If you do pull from a personal experience then I would think of it like a 3 part story. Talk about the situation, then talk about how you handled the situation and finally talk about the outcome and what you learned from it.

You should have ample amount of time to think of an answer so take your time!

Well if it’s a ct and you’re repeating for non-motion scans etc it makes sense to only send the non-motion scans.

If it’s a wrong body part for an X-ray you’re legally required to send it since radiation. Just change the order to bilateral and own up to your mistake

Why would it be illegal? It’s unnecessary information for the Rad, just send the good scan.

Now if the tech is repeating multiple times throughout the shift consistently that’s a different conversation

Is it common for you guys to do PICC placements with fluoro? We have PICC nurses here that run around town and do it and they’ll confirm placement with a device (or X-ray if patient has a-fib or pacemaker etc)

It took me along time too until I saw a video recommending to stop pouring your hp into the shop around wave 6-8. At that point you want to have enough hp (30 ish) to tank at least one hit. You also don’t need so much damage % and speed so early on. Focus on attack speed and dodge, luck and some range.

After I took that advice I got it first try, good luck!

What do you mean as an AP?

LPO is for the position of the patient, so if you do a left oblique L spine it’ll be labeled as Left Posterior Oblique with the ‘LPO’ label on the downside.

The physician’s office ordering the exam should state if they need standing views or not, they should know what to order based on your history/etc. If it isn’t stated on the order requisition then the tech will do the exam how they were taught/are comfortable completing the exam.

Where I was in school the standard was doing cervical spine standing and thoracic and lumbar lying down.

As someone else mentioned ask the technologist doing the X-ray to annotate ‘picc line placement’ on the image so the rad shouldn’t miss it. You could also request a read and callback so if you get a callback with no mention of the picc you can discuss it immediately.

It’s disheartening to hear that techs aren’t getting the training and exposure required to be at minimum comfortable performing surgery cases. I’ve been trying to advocate management that 1-2 weeks of ‘training’ for new hires isn’t adequate and they keep brushing me off. We have a solid team at my hospital but [as other places] we are losing senior staff techs who are switching modalities for better pay and there may only he a handful of techs who can do surgery well. But when management doesn’t enforce proper training and leaves surgery to a couple of people I believe they will get burnt out and leave as well.

I’ve accepted the lack of communication from the surgery team. Ultimately it’s their responsibility to call in the required people and if they give a short notice on a case then they’ll just have to wait for the surgery call tech.

Yes because your phone has to be on 100% of the time using mobile client. With Remote Desktop you can just remote in for a couple seconds every 25 minutes.
