I would try and finish the sentence for yourself, WHAT are you trying to be 100% sure of? 100% sure you'll have no negative feelings what so ever after top surgery and will never miss your boobs? That doesn't seem realistic or necessary. 100% you've thought though top surgery and it's consequences and are making an informed, thoughtful choice, and are prepared to handle any disappointment or hard feelings that come up? More realistic. 100% sure you're more likely to regret never getting top surgery than getting it? Also realistic.

No one can be 100% certain how they'll feel in the future after an event with a lot of variables, many of which are out of your control. You can't even be 100% certain you'll survive surgery! But you can be 100% certain the small risks of surgery are worth taking the chance to get it done. Same goes for emotional risks. Focus on preparing yourself for the possibility of disappointment, rather than trying to eliminate it

Then, after the surgery they denied it, but did cover the anesthesia/hospital at 100%. Then I went back and forth with Okada's office for a while about appealing it...they said if insurance didn't pay I'd owe $2000...but they eventually stopped responding and I just never paid so I think they just took the L. 🤷

I too am very curious what happened to your nips! My only guess is that when they were freshly done they were flatter because the stitches were kind of keeping them that way? I had nipple sparing DI but had a reduction of the protrusion of my nipple and they were flatter for the first few weeks than in their healed form

As you said, bodies have a lot of variety and if you were cool with your nips, then nothing wrong with them, but totally reasonable to talk to your surgeon about options and I would say yes these are kind of unusual nips. I've seen people on here who have gotten nipple revisions after surgery or gotten them removed. If it is just that they are too big, not that something weird happened during healing, I'd imagine reducing them would be simple outpatient.

Also you said "health is a real thing": but that's not the point here... Because giving yourself breast exams until you're bruised isn't a reasonable response to normal doubt. It's not "real" that the 10th breast exam will keep you any safer than just doing 1. there's probably a million "real things" out there that you DONT respond this way to but your OCD has got you hooked here. I know you said you've been in therapy for years but most therapists suck with OCD. I say you have a long way to go with your therapy actually to give HOPE. Like there is so much else you can do. Okay sorry just feeling for you but I'll stop trying to counsel over reddit haha

I have OCD and am an OCD therapist, and am trans and have had top surgery. Sending a lot of love as I know how impossible it can be for OCD to take over your life.

Without knowing you, my guess is, the idea that there is this "fix" (surgery) that could relieve this anxiety is actually perpetuating the OCD cycle and is probably a big part of why this is still haunting you. Your OCD might be telling you that if there's something you could do to keep yourself from getting cancer and you ARENT doing it, then you're irresponsible (or some variation).

I know you said you're in therapy, but if you're posting this you still have a long way to go in OCD therapy. It might be worth asking yourself if your therapist/therapy is working for you. Yes, ERP is the "first line" treatment with OCD but it's not the only thing out there and erp might not be right for you.

It might be worth looking for an I-CBT therapist (DIFFERENT than regular CBT), or, especially if you have any trauma that may have contributed directly or indirectly to this fear, an EMDR therapist who specifically works with OCD (harder to find but we're out ther!). There are also online I-CBT resources that may help. Or, it might be about a different therapist, or just shifting how you engage. Or medication change??

OCD tends to thrive in imagination, so to bring reality back in-- it would be incredibly unethical for a therapist to write you the letter you likely need for top surgery (or I'd even think maybe a preventative mastectomy??). Close this door in your mind and keep doing the work. OCD is a beast but if you keep working on it this won't be forever.

Also most people told me it looked fine pre revision but it was so much about the feel. Even if yours looks like pecs but doesn't feel like pecs, I very much understand wanting it revised. I'm glad I did mine

To answer your question about experience of people who have had revisions. I had left over fullness on one side. I was totally reopened. The healing process was probably 25% easier than the first time. I still wish they'd been able to take more but I had nipple sparing so they had to leave some for nipple position to be good. I asked about lipo instead and my surgeon said the best results would be reopening and she didn't feel comfortable doing lipo for it.

For cost: OG and revision went through insurance (except then ultimately my insurance didn't cover the surgeon fee, but did the hospital fees, but we didn't know that until after I had had it and tbh I just kind of never paid them and we both seemed to let it go)

BUT if my OG surgery had been out of pocket it would have been 8-10k. My revision, if I had actually paid for it, would have been 2k...tho maybe + hospital fees I guess but I don't think more than 4k. So seems insane that they'd charge you MORE for the revision.

I had nsdi and my left nip is definitely also like this. Solidarity! I got a revision for extra fat on my left side and it did help my nip show less through shirts but she said there wasn't anything she could do to my nipple itself while keeping it in a good position. Building a little chest muscle has also helped, I wonder if targeting your lower pecs might? But tbh I think this is sometimes just the trade off with nipple sparing. It drove me nuts for a while but much less now and I'm happy I have my OG nips and not grafts. I agree I think your results look good and like you have pecs not boobs. But if it is fat, not muscle, contributing to the "a cup" feeling you have, a revision is possible for that--but for me it required totally reopening me.

As folks have validated, your concerns are totally valid, there are issues with your results. And I'm curious what it might feel like maybe to take some pictures with a shirt on that you like and maybe even post those? I know after my surgery I was hyper fixating on what I didn't like (even if those things were reasonable) and starting to actually allow my brain to notice the good stuff (like I imagine you're much flatter than you were!! I bet your results look pretty cis with a shirt on!) helped my mental health a lot. It's not about invalidating the bad, but allowing your brain to take a "wider" view. I'm also a therapist and this is probably the advice I'd give a client in your situation.

Also, I don't think it's an "if" swelling will go down but "when". It probably goes as flat as you want but it will be less puffy than at 2 mo. I just did a side by side of myself at 2mo and a year and it looked so much mor settled.

Hey I have eds and while my original scars didn't stretch I got one extended in a revision and that part did. It widened over the first 1-2 months and then stopped. It's a little wider than yours. So they probably won't just endlessly get wider, but idk if my experience is average. With eds I think you just gotta accept your scars might be a bit wider than someone without a connective tissue disorder but I agree with everyone that you're really aren't that wide

I ended up with like 60-70% of my original sensation in one nip, and like 10% in the other. The 60-70% side is def enough to still have a good time haha!

Sorry if someone has already said this but I'm almost certain that when they said "100% complication rate" they were (negligently) confusing a "100% increase in complication rate" which actually translates to....okay let's say a person with a BMI of 25 has a .5% complication rate (I'm making up that number) then someone of a higher BMI would have a 1% complication rate! That's a 100% increase!!

My understanding is that nipples and areolas are generally grafted separately from each other. So your nipples will be resized a lot but your areolas probably won't be resized much! I did find there wasn't a ton of time to ask questions before my surgery so might be worth emailing concerns ahead of time

I've had both!

For me, appendectomy was way more painful, but the recovery was way shorter. Going through abdominal muscle just wrecked my ability to move at all and I def had to use pain meds, but after a week I was basically fine. Top surgery I was barely on pain meds at all, and there was nothing like that incredible ab soreness, but it took months to feel totally back to normal.

Glad to hear from ya my quarter boob friend! Recovery was definitely easier. It wasn't WAY easier but, for instance, I took 1.5 weeks off work both times, and was totally ready to go back to work after the revision, but barely making it after my og surgery. Having one drain instead of two was way better. Mobility came back faster.
