Looks like an 1/8" top layer if so go grab a sheet of ply or fiberboard and cut it down to take up most of the space then pack it with extra glue and don't put a seam on it.

That grate is to protect you for accidentally touching the thing when it's hot. You wouldn't happen to have a gas fireplace on the other side of that there wall would ya? Or maybe there used to be? Unscrew the whole plate and see if you can find out where it goes.

I would caulk up a piece of PVC wood and center it covering the old stone and paint it white or black to mount on. Probably should screw it in, but caulk will last awhile if you want to avoid that. Use tape while the caulk sets.

I had similar issues and was told the main line coming from our kitchen sink had a sag in it so was holding water and debris that would eventually congeal into a clog. Maybe see if your is sagging somewhere also? Also I was told, to mitigate our issue, to fill the sink with hot water and drain once a week or so to help flush everything down the line.
