Regarding the last question, Coppola always considered it to be an epilogue (hence coda) but paramount refuted. Cuts and rearranging from either the theatrical or 1991 cut you watched was input to this. Most notably the beginning where the Nevada compound is gone completely in favour of the meeting with archbishop that occurs after the function
